摘要
目的探讨超声对胎儿静脉导管异常的诊断价值;方法回顾性分析产前超声诊断的15例胎儿静脉导管异常临床资料,总结其声像图特点,分析原因。结果15例确诊胎儿静脉导管异常中,1例为静脉导管缺失伴持续性右脐静脉;14例为静脉导管频谱异常,即A波反向,其中胎儿心率失常并孕妇免疫系统疾病1例,三尖瓣异常2例,肺动脉瓣狭窄1例,卵圆瓣膨胀瘤5例,动脉导管异常2例,法洛斯四联症1例,复杂先天性心脏病2例。7例引产,1例失访,1例出生后手术治疗,6例建议提前终止妊娠。随访婴儿,心脏超声未发现异常。结论超声检查能准确诊断胎儿静脉导管的发育及频谱异常,确定胎儿先天性心脏病种类,对胎儿的治疗及转归具有重要评估价值。
Objective To evaluate the diagnostic value of ultrasonography for abnormal fetal venous catheter.Methods The clinical data of 15 cases of fetal venous catheter abnormality diagnosed by prenatal ultrasound were analyzed retrospectively.The characteristics of sonogram were summarized and the causes were analyzed.Results In 15 cases of fetal venous catheter abnormalities,1 case was the absence of venous catheter with persistent right umbilical vein,and 14 cases were the abnormal frequency spectrum of venous catheter,i.e.a wave reverse.These 14 cases included 1 case of fetal heart rate abnormality and maternal immune system disease,2 cases of tricuspid valve abnormality,1 case of pulmonary valve stenosis,5 cases of oval valve expansion tumor,2 cases of arterial catheter abnormality,1 case of tetralogy of Fallot,and 2 cases of complex congenital heart disease.There were 7 cases of induced labor,1 case of lost visit,1 case of operation after birth,and 6 cases of early termination of pregnancy.During follow-up,no abnormality was found by echocardiography.Conclusions Ultrasound can accurately diagnose the development and spectrum abnormality of fetal venous catheter,and determine the type of fetal congenital heart disease,which has an important evaluation value for the treatment and outcome of the fetus.
