摘要
目的探讨血浆血红蛋白、血小板计数、D二聚体及凝血4项检测在孕晚期产妇产后出血预测中的临床应用价值。方法选取该院分娩并于孕晚期发生产后出血的172例产妇作为研究组,另选取同期孕晚期未发生产后出血的186例产妇作为对照组。比较分析两组产妇血浆血红蛋白、血小板计数、D—二聚体及凝血4项的变化情况。结果研究组产妇血红蛋白(Hb)、血小板计数(PLT)和D-二聚体(D-D)水平均明显高于对照组(P<0.05)。研究组产妇凝血4项中凝血酶原时间(PT)和活化部分凝血活酶时间(APTT)水平均明显高于对照组(P<0.05);而研究组产妇血浆纤维蛋白原(FIB)水平则明显低于对照组(P<0.05);经Logistic回归分析,Hb、PLT、D-D、PT、APTT及FIB是产妇产后出血的独立预测因素(P<0.05)。结论血浆血红蛋白、血小板计数、D-二聚体以及凝血4项检测对于孕晚期产妇产后出血的预测具有重要的参考价值,并能够为预防和治疗产后出血提供依据。
【Objective】To investigate clinical value of blood hemoglobin,platelet count,D-dimer and coagulation for predicting postpartum hemorrhage in late pregnancy.【Methods】A total of 172 parturients with postpartum hemorrhage in late pregnancy in our hospital were selected as study group and another 186 parturients without postpartum hemorrhage in late pregnancy as control group.The levels of blood hemoglobin(Hb),platelet count(PLT),D-dimer(D-D),and four items of blood coagulation of the two groups were compared.【Results】The levels of Hb,PLT and D-D of study group were significantly higher than those of control group(P<0.05).The levels of prothrombin time(PT)and activated partial thromboplastin time(APTT)of study group were significantly higher than those of control group(P<0.05).The levels of fibrinogen(FIB)of study group were significantly lower than those of control group(P<0.05).Logistic regression analysis showed that Hb,PLT,D-D,PT,APTT and FIB were independent predictors of postpartum hemorrhage(P<0.05).【Conclusion】The detection of plasma hemoglobin,platelet count,D-dimer and coagulation possess important reference value in the prediction of postpartum hemorrhage in late pregnancy and it can provide basis for prevention and treatment of postpartum hemorrhage.
