摘要
目的探讨不同损伤机制所致青少年胫骨结节撕脱骨折的临床特点。方法纳入2013年6月至2016年11月中国中医科学院广安门医院南区收治的42例青少年胫骨结节撕脱骨折患者,根据损伤机制的不同分为低应力组12例(跑步时无明确外伤史)和高应力组30例(运动时滑倒或跌倒或碰撞史),比较两组患者的年龄、身高、体重、体重指数(body mass index,BMI)、BMI百分位数、骨折类型以及并发症发生情况。结果两组患者年龄、性别、身高、骨折侧别、骨折类型、吸烟人数和并发症发生情况比较差异无显著性(P>0.05)。低应力组患者的体重(P=0.0133)、BMI(P=0.0232)和BMI百分位数(P=0.0121)均明显高于高应力组,差异有显著性。结论BMI明显升高或降低可能是青少年运动中无明确创伤下发生胫骨结节撕脱骨折的危险因素。
Objective To investigate the characteristics of avulsion fractures of tibial tubercle in adolescents with different injury mechanisms.Method Forty-two patients with avulsion fracture of tibial tubercle were included in this study,including 12 cases(10 males and 2 females)in the low stress group(no history of trauma during running)with the median age of 13 years(range,11 to 17 years),and 30(22 males and 8 females)cases in the high stress group(history of slide,fall or collision during exercise)with the median age of 15 years(range,12 to 18 years).The age,height,weight,body mass index(BMI),BMI percentile,fracture type and complications of the two groups were compared.Result There were no significant differences in age,gender,height,facture’s side,fracture type,smoking and complications between groups(P>0.05).However,the body weight(P=0.0133),BMI(P=0.0232)and BMI percentile(P=0.0121)in the low stress group were significantly higher than those in the high stress group.Conclusion The significant increase or decrease of BMI compared with the same age may be the risk factors of avulsion fracture of tibial tubercle in adolescents without definite trauma during running.
