髋臼边缘负重区全缺损结构性植骨的不同固定方式对人工全髋关节置换术后植骨块初始微变形程度影响的有限元分析 被引量:3

摘要 目的利用三维有限元技术研究发育性髋关节发育不良(Development Dysplasia of the Hip,DDH)髋臼边缘负重区全缺损人工全髋关节置换术后三种固定方式对植骨垫块变形程度的影响。方法利用三维有限元技术建立骨盆模型模拟发育性髋关节发育不良髋臼边缘负重区全缺损全髋关节置换术后模型,在单足站立位模型上就双螺钉固定、单重建钢板固定和螺钉加重建钢板固定三种垫块固定方式进行有限元研究分析。结果通过有限元分析结构性植骨术后最大微变形程度,分析结果显示螺钉固定组最大变形度为0.277 mm,单重建钢板固定组为0.0333 mm,螺钉加重建钢板固定组最大变形度为0.0077 mm,螺钉加重建钢板固定组最大变形度明显小于其余两组。结论在髋臼边缘负重区全缺损情况下钢板加螺钉的结构性植骨固定方式在术后对植骨块变形程度影响最小。 Objective To investigate the effect of three fixation methods on the deformed degree of bone graft pad after total Hip arthroplasty for the total defect of the weight-bearing region of the acetabular margin of development dysplasia of the Hip(DDH)by using three-dimensional finite element technique.Methods The three-dimension finite element technology was used to establish a pelvis model to simulate total hip replacement model for developmental dysplasia of total defect of acetabulum edge weight bearing area after total Hip arthroplasty.On the single foot standing model,a finite element analysis was applied to double screw fixation,single reconstruction plate fixation and screw and reconstruction plate fixation.Results The maximum deformation degree after structural bone grafting was analyzed by using the finite element method.The results showed that the maximum deformation degree of the screw fixation group was 0.277 mm,the single reconstruction plate fixation group was 0.0333 mm and the screw and reconstruction plate fixation group was 0.0077 mm.The maximum deformation degree of the screw and reconstruction plate fixation was significantly less than that of other two groups.Conclusion In case of total defect in the weight-bearing area of the acetabular margin,the screw and reconstruction plate fixation has the least effect on the degree of bone graft deformation after surgery.
