摘要
目的探讨督导家庭肺康复对80岁以上慢性阻塞性肺疾病(COPD)患者的治疗意义。方法收集2018年6月至2019年6月在我院住院80岁以上确诊为COPD的67例患者,按照住院顺序进行编号,随机分为对照组(32例,健康教育)和康复组(35例,健康教育+康复训练),随访6个月,比较两组患者治疗效果。结果干预后两组COPD患者生活质量评估问卷(CAT)、改良英国医学研究会呼吸困难量表(mMRC)、焦虑自评量表(SAS)、抑郁自评量表(SDS)评分及急性加重和住院次数均较干预前降低,6分钟步行距离(6MWD)较干预前增加(P<0.05)。6个月时康复组6MWD明显高于对照组,其他4个评分、急性加重及住院次数均明显低于对照组,差异均有统计学意义(P<0.05)。结论督导家庭肺康复依从性高,能显著改善COPD患者的呼吸困难、缓解焦虑抑郁,增加运动耐力,对稳定病情、提高生活质量起到了积极的促进作用。
Objective To investigate the therapeutic effect of supervising home-based pulmonary rehabilitation on chronic obstructive pulmonary disease(COPD) patients aged 80 and over.Methods 67 patients who were hospitalized in our hospital from June 2018 to June 2019 with COPD over 80 years of age were randomly divided into control group(32 cases,health education) and rehabilitation group(35 cases,health education+rehabilitation training),followed up for 6 months to compare the treatment effect of 2 groups.Results Compared with before intervention,the score of CAT,mMRC,SAS,SDS,and the times of acute exacerbation and hospitalization were significantly decreased and 6 MWD was significantly lengthened after intervention in the two groups(P<0.05).The improvement degree of CAT,mMRC,6 MWD,SAS,SDS and the less degree of acute exacerbation and hospitalization times in the rehabilitation group were all superior to those in the control group,the difference was statistically significant(P<0.05).Conclusion Supervising home-based pulmonary rehabilitation can significantly improve dyspnea,relieve anxiety and depression,and increase exercise endurance.It has a positive effect on stabilizing the disease and improving the quality of life.
