摘要
消化道疾病中慢性胃炎、消化性溃疡、胃癌等疾病具有共同的致病因素即幽门螺旋杆菌(Hp)感染,但是反流性食管炎中Hp阳性的患者,其病情却较轻。目前,"慢性浅表性胃炎-萎缩性胃炎-肠上皮化生-非典型增生-胃癌"模式已经得到多数临床学者认可,在这一系列演变过程中,Hp的感染在不同阶段、不同程度上起着促进作用。本文从Hp的微生物学特性、致病机制方面,慢性胃炎、消化性溃疡、胃癌、口腔炎症和反流性食管炎等消化道疾病与Hp阳性的关系,上述疾病的预防、诊断、治疗和Hp感染的根除等方面进行综述。本文综述目的是预防Hp的感染,感染后及时诊断和治疗,防止其病情反复发作和恶化。而且Hp被根除后慢性胃炎患者局部炎症反应减轻或恢复正常,消化性溃疡患者愈合加快、复发率降低,随之胃癌的发病率也降低。
Chronic gastritis, peptic ulcer and gastric cancer in digestive tract diseases share the same pathogenic factor: Helicobacter pylori(Hp) infection. However, patients with positive Hp in reflux esophagitis have a milder condition. At present, "chronic superficial gastritis-atrophic gastritis-intestinal metaplasia-atypical hyperplasia-gastric cancer" model has been recognized by most clinical scholars, in this series of evolution process, Hp infection plays a promoting role in different stages and different degrees. This article reviews the microbiological characteristics and pathogenesis of Hp, the relationship between the positive of Hp and gastrointestinal diseases,such as chronic gastritis, peptic ulcer, gastric can cer, oral inflammation and reflux esophagitis,the prevention, diagnosis, treatment of these diseases and the eradication of Hp infection,etc. The purpose of this review is to prevent Hp infection, timely provide diagnosis and treatment after infection, and prevention of recurrence and deterioration of the disease. Moreover, after the eradication of Hp, the local inflammation in patients with chronic gastritis decreases or returns to normal, the healing of peptic ulcer patients accelerats, the recurrence rate decreases, and the incidence of gastric cancer also decreases.
