摘要
目的:观察补肾养肝活血汤加减联合治疗寻常痤疮的效果及对皮肤生理指标的影响。方法:纳入我院2018年1月至2019年1月期间82例寻常痤疮患者进行研究,采用随机数字表法随机分为观察组与对照组各41例,对照组予以西医常规治疗,观察组患者在此基础上联合补肾养肝活血汤加减治疗。比较两组患者治疗总有效率,以及治疗前、治疗8周后VISIA面部图像分析(毛孔、面部红色区、紫质、棕色斑)、皮肤生理指标[角质层含水量、经表皮失水率(TEWL)、皮肤颜色指数黑素指数(MI)、红斑指数(EI)、皮肤弹性指数(R2)]以及治疗期间不良反应发生情况。结果:治疗8周后,观察组患者的临床总有效率明显高于对照组(P<0.05);对照组患者皮肤面部红色区、紫质、棕色斑VISIA数值均较治疗前降低,观察组毛孔、面部红色区、紫质、棕色斑VISIA数值均较治疗前降低,且观察组显著低于对照组,差异均有统计学意义(P<0.05);两组患者角质层含水量、MI较治疗前差异无统计学意义,TEWL、EI均较治疗前降低,且观察组低于对照组,R2均较治疗前升高,且观察组高于对照组,差异均有统计学意义(P<0.05);治疗期间,对照组出现2例肝功能异常,对症治疗后恢复,观察组未出现尿常规、肝肾功能异常等不良反应。结论:补肾养肝活血汤加减联合治疗寻常痤疮可增强疗效,有效改善皮肤生理状况,且不增加不良反应。
Objective:To study the effect of using Modified Bushen Yanggan Huoxue Decoction in treating acne vulgaris and its influence on skin physiological indexes.Methods:A total of 82patients with acne vulgaris in our hospital from January 2018to January 2019were enrolled in the study.They were randomly divided into observation group and controlled group according to the random number table method,with 41cases in each group.The controlled group was given routine Western medicine therapy,and the observation group was combined with Modified BushenYanggan Huoxue Decoction on this basis.The total effective rate of treatment,and VISIA facial image analysis(pores,facial red area,sclererythrin,drab-colored patch),skin physiological in-dexes[water content of stratum corneum,transepidermal water loss rate(TEWL),skin color indexes of melanin index(MI),erythema index(EI),skin elasticity index(R2)]before treatment and after 8weeks of treatment and the occurrence of adverse reactions during treatment were compared between the two groups.Results:After 8weeks of treatment,the total clinical effective rate in the observation group was significantly higher than that in the controlled group(P<0.05).The VISIA values of skin facial red area,sclererythrin and drab-colored patch in the controlled group were lower than those before treatment,and the VISIA values of pores,facial red area,sclererythrin and drab-colored patch in the observation group were lower than those before treat-ment,and the values in the observation group were significantly lower than those in the controlled group(P<0.05).There were no significant differences in the water content of stratum corneum and MI in the two groups compared with those before treatment,and the TEWL and EI were lower than those before treatment,and the indexes in the observation group were lower than those in the controlled group.The R2was higher than that before treatment,and the index in the observation group was higher than that in the controlled group(P<0.05).During treatment,2cases of liver function abnormality occurred in the controlled group,and re-covered after symptomatic treatment.No adverse reactions such as urine routine abnormality and liver and kidney dysfunction oc-curred in the observation group.Conclusion:Modified BushenYanggan Huoxue Decoction for treating acne vulgaris can enhance the efficacy and effectively improve the skin physiological condition without increasing the adverse reactions.
