摘要
目的探讨分析下颌发育过度患者予以下颌支矢状骨劈开术(bilateral sagittal splint ramus osteotomy, BSSO)正颌治疗的疗效及对患者ANB、SNB等指标影响效果。方法选取威海口腔医院2014年6月-2018年12月收治64例下颌发育过度患者,对患者进行BSSO正颌治疗,分析治疗效果,通过X线头影测量治疗前、后患者L1-NB角、U1-SN角、U1-L1角、L1-MP角、Y轴角、SNA、SNB、ANB、NA-PA以及NP-FH等指标情况。结果纳入患者的BSSO术治疗后整体疗程结束(16.2±3.3)个月,患者口腔术后的咬合关系良好,且畸形侧貌转变为直面型;患者治疗后SNB相比治疗前明显更小(P<0.05);患者治疗后ANB、NAPA角绝对值明显变小(P<0.05);患者治疗后L1-NB角、L1-MP角相比治疗前均增大,而U1-L1角相比治疗前相对减小(P<0.05)。结论针对下颌发育过度患者行BSSO正颌治疗,在术前、术后结合方丝弓技术正畸治疗能有效保障手术疗效,且在术后改善ANB、SNB指标,该术式具有临床应用价值。
Objective To investigate and analyze the effect of orthognathic treatment with bilateral sagittal splint ramus osteotomy,(BSSO) in patients with overdeveloped mandible and its effect on the ANB, SNB and other indicators of patients. Methods We selected 64 patients with mandibular overgrowth from June 2014 to December 2018 in Weihai Stomatological Hospital. The patients were treated with orthognathic treatment by BSSO and analyzed treatment effect. Measurement of L1-NB angle, U1-SN angle, U1-L1 angle, L1-MP angle, Y-axis angle, SNA, SNB, ANB, NA-PA and NP-FH and other indicators before and after treatment by X-ray cephalometric. Results The end of overall treatment period(16.2 ± 3.3) months after thepatients treated with BSSO operation, the occlusion relationship of the patient after oral surgery was good, and the deformed side profile changed to a straight face;after the patient’s treatment, ANB, SNB, NP-FH and NAPA Compared with before treatment, it is significantly smaller(P < 0.05);after treatment, the L1-NB angle and L1-MP angle are increased compared with before treatment, while the U1-L1 angle is relatively reduced compared with before treatment(P < 0.05). Conclusion BSSO orthognathic treatment for patients with overdeveloped mandible, orthodontic treatment combined with square wire arch technique before and after surgery can effectively guarantee the surgical efficacy, and improve the ANB and SNB indicators after surgery,and the operation has clinical value.
