摘要
吸烟对支气管哮喘(简称哮喘)的发病、临床表现、治疗及预后有着显著的影响。本综述通过文献回顾及整理,总结吸烟对哮喘的影响。研究显示吸烟人群的哮喘发病率显著升高;在哮喘患者中,吸烟者比不吸烟者的哮喘控制程度更差、对糖皮质激素治疗的反应更弱,其机制可能与吸烟及相关的慢性气道炎症改变了哮喘的内型有关。戒烟可以在一定程度上改善哮喘患者的症状及肺功能,对于哮喘患者需要加强戒烟干预,且在今后哮喘治疗的临床试验中需要更多关注吸烟哮喘患者这一表型。
Smoking has significant impact on the onset,clinical manifestations,therapeutic outcomes and prognosis of asthma.Evidence of the influence of smoking on asthma through literature was summarized in this review.Significant increase of asthma prevalence among smokers had been shown in many studies.Smokers experienced worse asthma control and less responsiveness to corticosteroids,compared to nonsmokers among asthmatic patients.Mechanisms of impact of smoking on asthma included altered airway inflammation and asthma endotype.Smoking cessation can improve symptoms and lung function among patients with asthma.Smoking cessation interventions need to be further strengthened among patients with asthma.Smoker asthmatic patients should be enrolled in clinical trials of effectiveness assessing in treatment of asthma,to identify the effectiveness for this special phenotype.
