摘要
目的探讨妊娠糖尿病患者糖化血红蛋白水平与母亲体质指数、新生儿体重和母婴结局的关联。方法选取2016年1月至2018年12月南方医科大学附属小榄医院诊治的80例妊娠糖尿病患者为研究对象,根据糖化血红蛋白(HbAlc)检测结果将其分为达标组(HbAlc≤6.5%)和超标组(HbAlc>6.5%),达标组45例患者,超标组35例患者,比较两组患者体质指数(BMI),娩出新生儿体重以及母婴结局;采用酶促化学发光免疫法测定两组患者甲状腺激素水平;分别应用疲劳严重度量表(FSS)、抑郁自评量表(SDS)、焦虑自评量表(SAS)评估两组患者疲劳、抑郁和焦虑情况。结果超标组患者妊娠前BMI、产前BMI均高于达标组患者(均P<0.05)。超标组患者娩出新生儿体重、巨大儿发生率均高于达标组患者(均P<0.05)。超标组患者T3、FT3、T4、FT4等甲状腺激素水平均低于达标组患者(均P<0.05)。超标组患者FSS评分、SDS评分、SAS评分均高于达标组患者(均P<0.05)。超标组患者早产、羊水过多、胎膜早破发生率均高于达标组患者(均P<0.05)。结论妊娠糖尿病患者糖化血红蛋白水平超标与产妇BMI、娩出新生儿体重增加相关;同时,糖化血红蛋白水平超标增加了妊娠糖尿病产妇的疲劳、焦虑和抑郁易感性,与不良母婴结局风险增加相关。
Objective To investigate the association of glycosylated hemoglobin with maternal body mass index,neonatal weight and maternal and infant outcomes in patients with gestational diabetes mellitus.Methods 80 patients with gestational diabetes mellitus admitted to our hospital from January 2016 to December 2018 were selected as the research objects.According to the results of glycosylated hemoglobin(HbAlc)test,they were divided into up-to-standard group(HbAlc<6.5%,n=45)and over-standard group(HbAlc>6.5%,n=35).Maternal body mass index(BMI),neonatal weight,maternal and infant outcomes was compared between the two groups.Chemiluminescence immunoassay was used to measure the levels of thyroid hormone in the two groups.Fatigue severity scale(FSS),Self-rating Depression Scale(SDS)and Self-rating Anxiety Scale(SAS)were used to evaluate the fatigue,depression and anxiety of the two groups.Results Pre-pregnancy and prenatal BMI were higher in the over-standard group than those in the up-to-standard group(all P<0.05).The incidence of neonatal weight and macrosomia in the over-standard group was higher than that in the up-to-standard group(all P<0.05).The levels of thyroid hormones such as T3,FT3,T4 and FT4 in the over-standard group were lower than those in the up-to-standard group(all P<0.05).The FSS score,SDS score and SAS score in the over-standard group were higher than those in the up-to-standard group(all P<0.05).The incidence of premature delivery,excessive amniotic fluid and premature rupture of membranes in the over-standard group was higher than that in the up-to-standard group(all P<0.05).Conclusions Excessive glycosylated hemoglobin levels in patients with gestational diabetes mellitus are associated with maternal BMI and birth weight gain of newborns.In addition,excessive glycosylated hemoglobin levels can increase the risks of maternal fatigue,anxiety and depression susceptibility and adverse maternal and infant outcomes.
