摘要
目的探讨循证护理联合早期干预在降低骨折患者深静脉血栓形成中的应用效果。方法选取我院(2016年6月至2019年6月)收治的下肢骨折患者110例,根据不同护理分为两组,两组患者均接受常规护理,密切注意患者肿胀、疼痛程度和皮肤颜色、温度,抬高患肢,加强活动等,对照组(n=55)接受常规护理,观察组(n=55)接受循证护理联合早期干预,对比两组患者D-二聚体、纤维蛋白原水平和DVT发生率以及护理满意度。结果观察组患者的D-二聚体(4.38±1.28)mg/L,纤维蛋白原(4.84±1.18)g/L;对照组患者分别为(8.21±1.40)mg/L、(5.57±1.46)g/L,两组患者D-二聚体和纤维蛋白原水平对比差异明显(t=14.9736、2.8839,P<0.05)。观察组DVT发生率1.82%(1/55),对照组DVT发生率9.09%(5/55),两组患者DVT发生率对比差异明显(χ^2=5.1240,P<0.05)。观察组患者总满意度96.36%(53/55),对照组患者总满意度85.45%(47/55),两组患者护理满意度对比差异明显(χ^2=7.1983,P<0.05)。结论循证护理联合早期干预在降低骨折患者深静脉血栓形成中的应用效果满意,能够显著降低深静脉血栓发生率,提供护理满意度,值得临床应用。
Objective To explore the effect of evidence-based nursing combined with early intervention in reducing deep vein thrombosis in fracture patients.Methods A total of 110 patients with lower extremity fracture admitted to our hospital from June 2016-June 2019 were divided into two groups according to different nursing.Both groups received routine nursing,paying close attention to the degree of swelling,pain,skin color and temperature,raising the affected limb and strengthening activities.The control group(n=55)received routine nursing,and the observation group(n=55)received evidence-based nursing combined with early intervention D-dimer,fibrinogen level,DVT incidence and nursing satisfaction were compared between the two groups.Results the levels of D-dimer and fibrinogen were(4.38±1.28)mg/L and(4.84±1.18)g/L in the observation group and(8.21±1.40)mg/L and(5.57±1.46)g/L in the control group,respectively,with significant differences between the two groups(t=14.9736 and 2.8839,P<0.05).The incidence of DVT was 1.82%(1/55)in the observation group and 9.09%(5/55)in the control group.The total satisfaction of the patients in the observation group was 96.36%(53/55),and that in the control group was 85.45%(47/55).The difference between the two groups was significant(χ^2=7.1983,P<0.05).Conclusion Evidence-based nursing combined with early intervention can significantly reduce the incidence of deep vein thrombosis in fracture patients,and provide nursing satisfaction,which is worthy of clinical application.
