序贯性血液净化治疗急性重度有机磷农药中毒并发多器官功能障碍综合征(MODS)的临床效果分析

摘要 目的研究序贯性血液净化治疗急性重度有机磷农药中毒并发多器官功能障碍综合征(MODS)的临床疗效,并对其应用价值进行评价。方法将2017年1月-2018年12月期间该院接收的48例急性重度有机磷农药中毒并发多器官功能障碍综合征患者作为研究对象,根据单盲分组法将其分成两个组别(参照组和研究组),参照组对患者进行常规治疗,研究组在常规治疗基础上给予患者序贯性血液净化,观察并对比两组患者的治疗效果和临床指标的变化。结果研究组患者死亡率为8.3%,参照组患者死亡率为33.3%,两组数据对比差异有统计学意义(χ^2=4.55,P<0.05);且研究组患者在清醒时间(7.98±1.92)h、机械通气时间(30.45±3.67)h以及住院时间(10.13±1.87)d方面的临床指标,明显优于参照组,差异有统计学意义(t=8.67、13.91、10.14,P<0.05)。研究组患者在TNF-α(69.76±8.11)ng/L、IL-6(89.72±14.99)ng/L、IL-10(16.22±4.01)ng/L方面的数据,明显优于参照组,差异有统计学意义(t=9.25、8.62、22.92,P<0.05)。结论应用序贯性血液净化治疗急性重度有机磷农药中毒并发多器官功能障碍综合征,能够明显改善患者临床症状和生命体征,降低患者死亡率,临床应用价值显著,值得研究推广。 Objective To study the clinical efficacy of sequential blood purification in the treatment of acute severe organophosphorus pesticide poisoning complicated with multiple organ dysfunction syndrome(MODS), and to evaluate its application value. Methods 48 patients with acute severe organophosphorus pesticide poisoning complicated with multiple organ dysfunction syndrome received in the hospital from January 2017 to December 2018 were divided into two groups according to the single blind grouping method(reference group and research group). The patients in the reference group were given routine treatment, and the patients in the study group were given sequential blood purification on the basis of routine treatment. The therapeutic effects and changes of clinical indicators were compared between the two groups. Results The mortality rate of the study group was 8.3%, and the mortality rate of the reference group was33.3%. The difference between the two groups was statistically significant(χ^2=4.55,P<0.05);and the study group was awake(7.98±1.92)h, mechanical ventilation time(30.45±3.67)h and hospitalization time(10.13±1.87)d clinical indicators, significantly better than the reference group, the difference was statistically significant(t =8.67,13.91,10.14,P <0.05). The data of TNF-α(69.76±8.11)ng/L, IL-6(89.72±14.99)ng/L, and IL-10(16.22±4.01)ng/L in the study group were significantly better than the reference group, and the difference was statistically significant(t=9.25, 8.62, 22.92,P<0.05). Conclusion Sequential blood purification for acute severe organophosphorus pesticide poisoning complicated with multiple organ dysfunction syndrome can significantly improve the clinical symptoms and vital signs of patients, reduce the mortality rate of patients, and has significant clinical application value, which is worthy of research and promotion.
