摘要
目的对不同分期糖尿病视网膜病变筛查中免散瞳眼底照相的应用价值进行探讨。方法 2017年1月-2019年5月选择糖尿病患者100例(200只眼),为所有患者行免散瞳眼底照相筛查并进行病理分期,统计不同病程患者发病率,对视网膜病变Ⅲ期及以上患者行荧光素眼底造影检查,对比不同诊断方法的诊断效果。结果免散瞳眼底照相检查:100例患者200眼睛中64眼存在不同程度视网膜病变,136眼无不适症状。视网膜病变分期:136眼为0期,24眼为Ⅰ期,15眼为Ⅱ期,11眼为Ⅲ期,3眼为Ⅳ期,7眼为Ⅴ期,4为Ⅵ期。病程<1年、1年≤病程<5年、5年≤病程<10年、10年≤病程<20年、病程≥20年糖尿病视网膜病变检出率分别为15.6%、17.1%、25.0%、42.6%、48.5%。200只眼睛糖尿病视网膜病变检出率为32.0%。免散瞳眼底照相和荧光素眼底造影诊断具有较高一致性(χ^2=0.00,P=1.00)。结论在糖尿病视网膜病变筛查中免散瞳眼底照相操作简便,和荧光素眼底造影诊断具有较高一致性。
Objective To explore the application value of free mydriatic fundus photography in screening of diabetic retinopathy at different stages. Methods from January 2017 to May 2019, 100 patients(200 eyes) with diabetes mellitus were selected, and all patients were screened with nondilated pupil fundus photography and pathological staging. The incidence of patients with different disease stages were counted. Fluorescein fundus angiography was performed on patients with retinopathy stage iii or above, and the diagnostic effects of different diagnostic methods were compared. Results in 100 patients, 64 out of 200 eyes had retinopathy of different degrees, and 136 eyes had no discomfort symptoms. Retinopathy in stages: 136 eyes of 0, 24 eyes for Ⅰ period, 15 eyes for Ⅱ period, 11 eyes for Ⅲ period, 3 eyes for Ⅳ period, 7 eyes of Ⅴ period, 4 only Ⅵ period. The detection rates of diabetic retinopathy were 15.6%, 17.1%, 25.0%, 42.6% and 48.5%, respectively. The detection rate of 200 eyes with diabetic retinopathy was 32.0%.There was a high consistency in fundus fluorescein and myotopic fundus radiography(χ~2=0.00, P=1.00). Conclusion in the screening of diabetic retinopathy, myopic fundus photography is easy to operate, and the diagnostic effect is consistent with that of fluorescein fundus angiography.
