摘要
目的研究分析环池CT三维重建变化与颅脑损伤患者预后的关系。方法选取东莞市石碣医院2017年7月-2019年6月收治的106例颅脑损伤患者的临床资料,采用颅脑CT对病情和预后进行评估。对患者进行为期3个月的随访,通过格拉斯哥昏迷评分(GCS)对预后的情况进行评价。分析环池分型与颅脑损伤预后的关系,采用Spearman对环池分型与GCS评分的相关性进行研究。结果环池分型1型、2型、3型、4型、5型、6型3个月后的死亡率分别为0.00%、5.88%、13.64%、24.00%、33.33%、41.67%,预后较佳率分别为92.86%、76.47%、45.45%、35.00%、23.81%、25.00%,预后不佳率分别为7.14%、23.53%、54.55%、65.00%、76.19%、75.00%,不同环池分型死亡率、预后较佳率和预后不佳率相比较,差异有统计学意义(P<0.05)。Spearman相关性分析法显示,环池分型与GCS评分呈负相关(r=-0.698,P<0.05)。结论环池CT三维重建变化能够反映颅脑损伤病情和预后情况,环池分型越高,GCS评分越低,预后越不佳。
Objective To study and analyze the relationship between three-dimensional reconstruction of ring-cistern CT and the prognosis of patients with craniocerebral injury. Methods Clinical data of 106 patients with craniocerebral injury admitted tothe hospital from July,2017 to June,2019 were selected,and craniocerebral CT was used to evaluate the condition and progno-sis. Patients were followed for three months and their prognosis was assessed by the Glasgow coma score(GCS). Spearman wasused to study the correlation between ring pool classification and GCS score. Results Mortality rates of ring pool parting type 1,2,3,4,5 and 6 after 3 months were 0.00%,5.88%,13.64%,24.00%,33.33% and 41.67%,better prognosis rate were92.86%,76.47%,45.45%,35.00%,23.81% and 25.00%,and poor prognosis rate were 7.14%,23.53%,54.55%,65.00%,76.19% and 75.00%. Different ring pool classification rate of mortality,better prognosis and bad prognosis rate were compared,and the difference was statistically significant(P<0.05). Spearman correlation analysis showed that ring-pool classification wasnegatively correlated with GCS score(r=-0.698,P<0.05). Conclusion CT three-dimensional reconstruction of ring cistern canreflect the condition and prognosis of craniocerebral injury. The higher the ring cistern classificationis, the lower the GCSscoreis, and the worse the prognosisis.
