基于“脾主肉,肾主骨”理论探讨OPG/RANK/RANKL信号通路与老年性骨质疏松的相关性 被引量:24

摘要 从中医辨证论治角度讲,肾主骨生髓,为先天之本,脾主肌肉、充四肢,为后天之源,先后天互资互生,共同维持人体正常生理机能。老年人各器官功能开始老化、衰退,脾肾日渐虚损,骨枯肉削,是骨质疏松症发生的关键。随着在分子生物学领域研究的不断深入,发现骨保护蛋白(OPG)/核因子-κB受体活化因子(RANK)/核因子-kB受体活化因子配体(RANKL)信号转导系统在骨代谢中起着关键的调控作用,尤其在骨骼和肌肉的微观调控方面越来越受到重视。骨骼和肌肉是运动系统的重要组成部分,本文基于"脾主肉,肾主骨"理论,结合OPG/RANK/RANKL信号通路在运动系统中转导作用,就脾肾亏虚与老年性骨质疏松症发病关系展开探讨,从分子学水平为从脾肾论治该病提供依据,以期更好指导临床。 From the perspective of TCM syndrome differentiation, the physiological role of bone and bone marrow depends on the kidney, which is the congenital foundation, and the physiological function of muscles and limbs depends on the spleen, which is the source of nurture. Congenital foundation and nurture tonify and endow each other, and together maintain the normal physiological functions of the human body. The functions of the elderly organs begin to age and decline, the spleen and kidney are gradually weakened, and the bones are cut, which is the key to the occurrence of osteoporosis. With the deepening of research in the field of molecular biology, it has been discovered that the osteoprotective protein(OPG)/nuclear factor-κB receptor activating factor(RANK)/nuclear factor-κB receptor activating factor ligand(RANKL) signal transduction system in bone metabolism plays a key role in regulation, especially in microscopic regulation of bones and muscles. Bone and muscle are important components of the motor system. Based on the theory of "spleen governing muscle and kidney dominating bone", and combined with transduction of OPG/RANK/RANKL signaling pathway in motor systems, this article reviewed the relationship between spleen-kidney deficiency and the pathogenesis of senile osteoporosis, and provided a basis for treating the disease from the perspective of spleen and kidney at the molecular level, so as to better guide the clinic.
