摘要
目的探讨使用低电压(85 KVp)、低对比剂剂量的条件下,肺动脉CTA的临床应用价值。方法连续收集64例临床怀疑肺动脉栓塞等肺部疾病而做肺动脉CTA病人,研究组使用碘克沙醇(270 mg/mL),85 KVp,共32例,对照组使用碘克沙醇(320 mg/mL),120 KVp,共32例;记录每组CT扫描自动生成的剂量参数,包括剂量长度乘积(DLP)和CT剂量指数(CTDIvol),比较两组图像参数情况,包括辐射剂量、肺动脉内CT值、图像对比噪声比(CNR)及信噪比(SNR)。结果使用双低剂量扫描图像质量主观评分及客观评分中的肺动脉内CT值评分两组差异无统计学意义(P>0. 05),图像质量满足临床诊断要求。在固定其它扫描参数前提下,管电压从120 KVp降至85KVp时,其CTDIvol、DLP分别下降了66. 33%、65. 03%,差异有统计学意义(P <0. 05)。结论64排螺旋CT肺动脉CTA应用双低剂量扫描是可行的,图像质量好能满足临床诊断的需要,同时能显著降低放射剂量。
Objective To explore the clinical value of pulmonary artery CTA under low voltage( 85 KVp) and low contrast dose. Methods Sixty-four patients with pulmonary CTA suspected of pulmonary embolism and other pulmonary diseases were divided into two groups. In the study group,32 patients were treated with ioxanol( 270 mg I/mL),85 KVp,and in the control group,32 patients were treated with ioxanol( 320 mg I/mL),120 KVp. Dose parameters automatically generated by CT scanning in each group were recorded,including dose length product( DLP) and CT dose index( CTDI).And image parameters,including radiation dose,CT value,image contrast noise ratio( CNR) and signal-to-noise ratio( SNR),were compared between the two groups. Results There was no significant difference in subjective score and objective score of image quality and CT value between the two groups( P > 0. 05),and the image quality met the requirements of clinical diagnosis. On the premise of fixing other scanning parameters,CTDIvol and DLP decreased by 66. 33% and 65. 03%respectively when the tube voltage decreased from 120 KVp to 85 KVp( P < 0. 05). Conclusion It is feasible to use double low-dose CT in 64-slice spiral CT pulmonary artery CTA. Good image quality can meet the needs of clinical diagnosis and significantly reduce the radiation dose.
