摘要
目的观察联合应用加压弹力袜(PES)和低分子肝素钠(LMWH)预防手术患者下肢深静脉血栓形成的临床效果。方法妇科盆腔手术患者120例随机均分为三组。PES组仅穿PES,LMWH组加用术后皮下注射LMWH 6000 U/d,IIPP组加用间歇充气压力泵(IIPP)。比较三组术后72 h下肢深静脉血栓发生率,分析下肢深静脉血栓发生的危险因素。结果120例患者中15例术后发生下肢深静脉血栓。LMWH组术后下肢深静脉血栓发生率低于IIPP组和PES组(2.5%vs.12.5%和22.5%)(P<0.05)。有下肢静脉曲张史、糖尿病史和手术时间长为妇科术后发生下肢深静脉血栓的独立危险因素(P<0.05),应用LMWH为妇科术后发生下肢深静脉血栓的保护因素(P<0.05)。结论有下肢静脉曲张病史、糖尿病史和手术时间长为下肢深静脉血栓发生的危险因素。联合应用PES和LMWH能有效降低妇科盆腔手术患者下肢深静脉血栓发生的风险。
Objective To observe clinical value of combined use of pressure elastic socks(PES)and low molecular weight heparin sodium(LMWH)in the prevention of lower extremity deep vein thrombosis(LDVT).Methods A total of 120 patients undergoing gynecological pelvic surgery was randomly assigned into three groups with 40 cases each.The patients in group PES were worn step-by-step PES.The patients in group LMWH were worn PES and subcutaneously injected with LMWH 6000 U daily.The patients in group IIPP were worn intermittent inflatable pressure pump.The incidence of LDVT in 72 hours after operation was compared among three groups.The risk factors for LDVT were analyzed.Results In 120 cases,15 patients developed LDVT after operation.The incidence of LDVT was lower in group LWMH than that in groups of IIPP and PES(2.5%vs.12.5%and 22.5%)(P<0.05).History of varicose veins of the lower extremities and diabetes,and longer operation time were the independent risk factors for LDVT after gynecological surgery(P<0.05),while use of LMWH was the protective factor for LDVT after gynecological surgery(P<0.05).Conclusion History of varicose veins of the lower extremities and diabetes,and longer operation time are the independent risk factors for LDVT after gynecological pelvic surgery.Combined use of PES and LMWH can effectively reduce the risk for LDVT after surgery.
