摘要
目的研究血清尿酸(SUA)水平与发生心肌纤维化的相关性。方法纳入56例于2014年1月至2019年5月在北京医院行心脏磁共振成像诊断为心肌纤维化(钆延迟强化阳性)的患者为病例组,按年龄、性别匹配56例心脏磁共振成像显示无心肌纤维化的同期住院患者为对照组,并回顾性地收集两组的基线SUA值和其他重要基线资料,采用logistic回归分析SUA是否为心肌纤维化的危险因素。结果心肌纤维化组与对照组匹配后,基线年龄、性别、血清三酰甘油、总胆固醇、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇和估测的肾小球滤过率及患高血压比例均无显著差异(均为P>0.05)。心肌纤维化组的SUA水平显著高于对照组[(7.14±2.10)mg/dl比(6.04±1.38)mg/dl,t=-3.269,P=0.002]。校正年龄、性别、体质指数、估测的肾小球滤过率、高血压、糖尿病和冠心病后,logistic回归分析显示患者基线SUA每增加1 mg/dl时,发生心肌纤维化的风险升高34.6%(OR:1.346,95%CI:1.015~1.785,P=0.039),基线有高尿酸血症的患者发生心肌纤维化的风险是基线无高尿酸血症者的4.098倍(OR:4.098,95%CI:1.445~11.623,P=0.008)。结论SUA水平升高是心肌纤维化发生风险增加的独立预测因素。
Objective To investigate the correlation between serum uric acid(SUA)levels and myocardial fibrosis.Methods Fifty-six patients with myocardial fibrosis diagnosed by cardiac magnetic resonance in Beijing Hospital were included in the case group,and age-and sex-matched 56 patients without cardiac fibrosis assessed by cardiac magnetic resonance served as the control group.The baseline SUA levels and other important baseline data of the two groups were collected retrospectively,and the risk of SUA for myocardial fibrosis was assessed by logistic regression analysis.Results There were no significant differences in the baseline age,gender and total cholesterol,triacylglycerol,low density lipoprotein-cholesterol,and high density lipoprotein-cholesterol,estimated glomerular filtration rate and the prevalence of hypertension between the patients with myocardial fibrosis and the controls(all P>0.05).The SUA levels in the case group were significant higher than those in the control group[(7.14±2.10)mg/dl vs.(6.04±1.38)mg/dl,t=-3.269,P=0.002].After adjusting for age,gender,body mass index,glomerular filtration rate,hypertension,diabetes and coronary heart disease,logistic regression analysis showed that the risk of myocardial fibrosis increased by 34.6%(OR:1.346,95%CI:1.015-1.785,P=0.039)for each 1 mg/dl SUA increment from baseline.The risk of myocardial fibrosis in patients with hyperuricemia at baseline was 4.098 times higher than those without hyperuricemia(OR:4.098,95%CI:1.445-11.623,P=0.008).Conclusions The increased SUA level is an independent predictor of increased risk of myocardial fibrosis.
