摘要
目的系统评价中药阿是穴外敷联合三阶梯止痛法治疗癌痛的疗效和安全性。方法计算机检索中国知网(CNKI)、中国生物医学文献数据库(CMB)、维普中文科技期刊全文数据库(VIP)、万方数据库、PubMed、The Cochrane Library数据库自建库至2019年8月关于中药阿是穴外敷联合三阶梯止痛法治疗癌痛的临床研究文献,选择符合要求的随机对照试验(RCT),采用RevMan5.3软件进行Meta分析。结果共纳入23项随机对照研究,纳入患者共计2170例。Meta分析结果显示,与单纯三阶梯止痛法治疗癌痛相比,中药阿是穴外敷联合三阶梯止痛法有效率更高(P<0.01),能够改善患者的生存质量(P<0.01),减少恶心、呕吐、便秘、头晕、头痛等不良反应的发生(P<0.01)。结论中药阿是穴外敷联合三阶梯止痛法治疗癌痛安全有效,能够改善患者的生存质量,减少不良反应的发生。虽然目前研究中药外敷联合三阶梯止痛法的RCT数量较多,但还需要更高级别的RCT进一步验证。
Objective:To evaluate the efficacy and safety of external application at Ashi point combined with three-step analgesia on cancer pain.Methods:CNKI,CMB,VIP,Wanfang database,Pubmed and The Cochrane library database were searched for clinical research literature on the treatment of cancer pain by external application at Ashi point combined with three-step analgesia by computer from establishment to August 2019.The eligible RCT was selected and meta-analysis was carried out with Revman 5.3 software.Results:23 RCTs including 2170 patients were included.External application at Ashi point combined with three-step analgesia was more effective than three-step analgesia only(P<0.01).It could improve the quality of life.It also reduce the incidence of adverse reactions including nausea and vomiting,constipation and dizziness and headache(P<0.01).Conclusion:External application at Ashi point combined with three-step analgesia can improve the efficiency of treating cancer pain,improve the quality of life of patients,and reduce the occurrence of adverse reactions.Although there are a large number of RCTs about it,higher level RCTs are still needed for further verification.
