摘要
目的探讨医联体分级诊疗经济性。方法采集某医联体中三级医院和二级医院2型糖尿病住院患者费用,运用Mann Whitney U秩和检验对比分析两院2型糖尿病患者基本情况和费用构成。结果患者基本情况中,年龄和住院天数,三级医院均明显高于二级医院。在住院费用结构组成中仅西药类费用两所医院没有明显差异。综合治疗服务费、治疗类费用、诊断类费用、中药类费用、耗材类费用以及总费用两所医院间均有明显差异。结论从医联体分级诊疗经济性优势角度,二级医院对于2型糖尿病诊疗比三级医院具有经济性优势。医联体应进一步推行分级诊疗政策,扩大分级诊疗病种范围,充分发挥医联体分级诊疗经济性优势。
Objective:To explore the economy of hierarchical diagnosis and treatment in medical alliance hospitals.Method:The expenses of inpatients with type 2 diabetes in hospitals of the second and the third grade of medical alliance were collected.Mann Whitney U rank sum test was used to compare the basic situation and expense composition of type 2 diabetes patients in both kinds of hospitals.Result:In patients’basic conditions,age and length of stay were significantly higher in the third grade hospitals than in the second grade hospitals.There was no significant difference between the two kinds of hospitals in the composition of hospitalization cost structure.But comprehensive treatment service fee,treatment fee,diagnosis fee,traditional Chinese medicine fee,consumables fee,and the total cost had obvious differences.Conclusion:From the perspective of economic advantages of hierarchical diagnosis and treatment of type 2 diabetes in medical alliance,hospitals of grade two have economic advantages over hospitals of grade three.Medical alliance should further promote the policy of hierarchical diagnosis and treatment,expand the scope of the diagnosis and treatment,and give full play to the economic advantages of the diagnosis and treatment of medical alliance.
