摘要
目的探讨冠心病患者糖耐量水平情况及其与血小板参数和冠状动脉病变程度的相关性。方法选取2018年7月—2019年7月收治冠心病126例,均进行口服葡萄糖耐量试验(OGTT),根据结果分为糖尿病组(A组)39例、糖耐量减低组(B组)44例、正常组(C组)43例。比较3组血小板参数、冠状动脉狭窄程度和病变支数以及Gensini评分情况。结果C组冠状动脉病变支数、狭窄程度、Gensini评分以及血小板平均体积、血小板分布宽度、大血小板比率低于A组和B组,血小板计数高于A、B组(P<0.05)。OGTT血糖水平与Gensini评分、血小板平均体积、血小板分布宽度及大血小板比率均呈正相关(r=4.264、3.602、3.846、3.701,P<0.05),而与血小板计数呈负相关(r=-3.546,P<0.05)。结论冠心病患者的糖耐量水平与血小板参数和冠状动脉的病变程度密切相关,即糖耐量水平越低,血糖水平越高,血小板平均体积、血小板分布宽度、大血小板比率越高,冠状动脉病变程度也越严重,因此应给予冠心病患者OGTT等相关检查,排除糖耐量水平降低对冠心病患者的治疗和预后造成的不良影响。
Objective To investigate the glucose tolerance level in patients with coronary heart disease(CHD)and its correlation with platelet parameters and severity of coronary artery disease(CAD).Methods A total of 126 patients with CHD admitted to our hospital from July 2018 to July 2019 were selected as subjects.All the patients underwent oral glucose tolerance test(OGTT).According to the results,the patients were divided into the diabetic group(group A,n=39),the reduced glucose tolerance group(group B,n=44),and the normal group(group C,n=43).Platelet parameters,stenosis of coronary artery and the number of diseased coronary artery as well as Gensini scores were compared between the three groups.Results The number of diseased coronary artery,degree of stenosis,Gensini score,mean platelet volume,platelet distribution width(PDW),and platelet-large cell ratio(P-LCR)in group C were significantly lower than those in group A and group B,while platelet count was significantly higher than that in group A and B(P<0.05).The blood glucose level of OGTT was positively correlated with Gensini score,mean platelet volume,PDW,and P-LCR(r=4.264,3.602,3.846,3.701,P<0.05),but negatively correlated with platelet count(r=-3.546,P<0.05).Conclusion The glucose tolerance level in patients with CHD is closely related to the platelet parameters and the degree of CAD.This indicates that the lower the glucose tolerance level is,the higher the blood glucose level,mean platelet volume,PDW and P-LCR are,and the more serious the CAD is.Therefore,patients with CHD should be given relevant tests such as glucose tolerance to rule out the adverse effects of decreased glucose tolerance on the treatment and prognosis of patients with CHD.
