摘要
目的探讨健脾化痰法治疗脾虚型支气管哮喘的临床疗效。方法回顾分析浙江中医药大学附属温州中医院2017年5月~2018年8月收治的60例脾虚型支气管哮喘的临床资料,随机分为治疗组和对照组。治疗组30例采用以健脾化痰法为治则的中药汤剂联合西药舒利迭治疗,对照组30例单用舒利迭治疗,比较两组中医证候疗效、单项中医证候积分、ACT评分。结果治疗组在总体疗效分布、显效率方面优于对照组(P<0.05),在临床症状改善方面,对比两组咳嗽、咳痰、胸闷、喘息等哮喘脾虚型主症方面,两者无显著差异(P>0.05),但在改善胃纳、乏力、便溏等次症方面,治疗组明显优于对照组(P<0.05)。在ACT评分方面,治疗组改善更明显(P<0.05)。结论健脾化痰法治疗脾虚型支气管哮喘疗效显著,能显著改善哮喘患者的全身症状,提高患者的ACT评分,说明中医在治疗支气管哮喘时有一定的优势,值得在临床推广。
Objective To explore the clinical effect of the spleen strengthening and phlegm reducing method in the treatment of bronchial asthma with spleen deficiency.Methods The clinical data of 60 patients with bronchial asthma with spleen deficiency treated in Wenzhou Hospital of Traditional Chinese Medicine Affiliated to Zhejiang University of Traditional Chinese Medicine from May 2017 to August 2018 were analyzed retrospectively and randomly divided into the treatment group and the control group.Thirty patients in the treatment group were treated with traditional Chinese medicine decoction based on spleen strengthening and phlegm reducing combined with western medicine sullipid,and 30 patients in the control group were treated with sullipid alone.The efficacy of TCM syndromes,single TCM syndrome points and ACT scores were compared between the two groups.Results The treatment group was superior to the control group in terms of the overall efficacy distribution and significant effective rate(P<0.05).In terms of improvement of clinical symptoms,the two groups were not significantly different in the main symptoms of asthma with spleen deficiency such as cough,sputum,chest tightness and wheezing(P>0.05),but the treatment group was significantly better than the control group in the improvement of gastric appetite,fatigue and stool symptoms(P<0.05).In terms of ACT scores,the improvement in the treatment group was more significant(P<0.05).Conclusion The method of spleen strengthening and phlegm reducing is effective in treating bronchial asthma with spleen deficiency,and it can significantly improve the systemic symptoms of patients with asthma and increase the ACT scores of patients.This shows that traditional Chinese medicine has certain advantages in the treatment of bronchial asthma and is worthy of clinical promotion.
