双钢板内固定加人工骨植骨治疗胫骨平台粉碎性骨折的临床效果和安全性 被引量:5

Clinical effects and safety of double steel plate internal fixation and artificial bone grafting in treatment of tibial plateau comminuted fractures

摘要 目的:观察双钢板内固定加人工骨植骨治疗胫骨平台粉碎性骨折的临床效果和安全性。方法:选取60例胫骨平台粉碎性骨折患者为研究对象,按随机数字表法分为对照组和观察组各30例。对照组实施传统手术,观察组实施双钢板内固定加人工骨植骨术。比较两组膝关节功能、日常生活活动能力量表(ADL)评分、手术相关指标水平及并发症发生情况。结果:治疗后,观察组Hohl膝关节功能评分及ADL评分均明显高于对照组,差异有统计学意义(P<0.05);观察组手术时间明显长于对照组,而住院时间、骨折愈合时间明显短于对照组,差异均有统计学意义(P<0.05);观察组并发症发生率为3.33%,明显低于对照组的26.67%,差异有统计学意义(P<0.05)。结论:采取双钢板内固定加人工骨植骨治疗胫骨平台粉碎性骨折患者效果好于单一的固定治疗,能有效改善预后,且安全性高。 Objective: To observe clinical effects and safety of double steel plate internal fixation and artificial bone grafting in treatment of tibial plateau comminuted fractures. Methods: 60 patients with tibial plateau comminuted fractures were selected as the research objects, and were divided into control group(n=30) and observation group(n=30) according to the random number table method. The control group underwent traditional surgery, while the observation group was given double steel plate internal fixation and artificial bone grafting. The knee function, activity of daily living scale score(ADL), surgery-related indicators and complications were compared between the two groups before and after the treatment. Results: After the treatment, the Hohl knee function score and ADL score in the observation group were higher than those in the control group, and the differences were statistically significant(P<0.05). The operation time in the observation group was significantly longer than that in the control group;the hospitalization time and fracture healing time were significantly shorter than those in the control group;and the differences were statistically significant(P<0.05). The complication rate in the observation group was 3.33%, which was significantly lower than the control group of 26.67%, and the difference was statistically significant(P<0.05). Conclusions: Double steel plate internal fixation and artificial bone grafting in the treatment of tibial plateau comminuted fractures is superior to single fixation. Moreover, it can effectively improve the prognosis with high safety.
