摘要
目的探究影响肛周脓肿合并糖尿病患者术后创面愈合质量的相关因素。方法选取2016年6月至2019年6月在本院接受手术治疗的肛周脓肿合并糖尿病患者166例为研究对象,评估患者术后7 d创面愈合质量。借助单因素分析和多因素Logistic回归分析法分析患者性别、年龄、体重、吸烟、血糖控制情况、术后坐浴时间、术后便秘(或便秘史)、饮食营养情况、机械刺激对创面愈合质量的影响。结果治疗7 d后,创面愈合良好者141例(84.94%),愈合不良者25例(15.06%)。经单因素分析和多因素Logistic回归分析显示,术后坐浴时间是影响肛周脓肿合并糖尿病患者创面愈合质量的保护性因素,吸烟、血糖控制情况、术后便秘(或便秘史)、饮食营养情况及机械刺激是影响创面愈合质量的危险因素(P<0.05)。结论肛周脓肿合并糖尿病患者术后创面愈合质量受术后坐浴时间、吸烟、血糖控制、便秘、术后饮食营养、创面处理方式等因素影响,术后应给予针对性干预,以改善患者创面愈合质量。
Objective To explore the related factors influencing the quality of postoperative wound healing in patients with perianal abscess and diabetes.Methods One hundred and sixty-six patients with perianal abscess and diabetes received treatment in our hospital from June 2016 to June 2019 were selected as the study objects,and the quality of wound healing at 7 days after operation was evaluated.By means of univariate analysis and multivariate Logistic regression analysis,the influences of gender,age,weight,smoking,blood glucose control and postoperative bath time,postoperative constipation(or constipation history),dietary nutrition and mechanical stimulation on wound healing quality were analyzed.Results After 7 days of treatment,141 cases(84.94%)had good wound healing and 25 cases(15.06%)had poor wound healing.The results of univariate analysis and multivariate Logistic regression analysis showed that time of sitting bath after operation was the protective factor of wound healing quality in patients with perianal abscess and diabetes,while smoking,blood glucose control,postoperative constipation(or history of constipation),dietary and nutrition and mechanical stimulation were the risk factors of wound healing quality(P<0.05).Concluion The quality of postoperative wound healing in patients with perianal abscess and diabetes is affected by factors such as postoperative sitting bath time,smoking,blood glucose control,constipation,postoperative diet and nutrition,wound management methods.After operation,targeted intervention should be given to improve the wound healing quality of patients.
