摘要
目的分析健脾利水方治疗肝硬化顽固性腹水的临床疗效及对肝肾功能的影响。方法92例肝硬化顽固性腹水患者按随机数字表法分为51例对照组和41例研究组,对照组采用常规治疗,研究组在对照组基础上联合健脾利水方治疗。比较两组临床疗效,治疗前后腹围、体重、24 h尿量、腹部B超指标、肝肾功能、生活质量和安全性。结果研究组总有效率高于对照组,比较有统计学差异(P<0.05)。治疗前,两组腹围、体重、24 h尿量、腹部B超指标、肝肾功能、生活质量比较无统计学差异(P>0.05);治疗后,两组腹围、体重、24 h尿量、腹部B超指标、谷草转氨酶(AST)、谷丙转氨酶(ALT)、总胆红素(TBIL)、尿素氮(BUN)、血肌酐(Cr)均下降,白蛋白(ALB)及生活质量均上升,研究组以上指标变化更明显,比较有统计学差异(P<0.05)。两组不良反应发生情况比较无统计学差异(P>0.05)。结论健脾利水方治疗肝硬化顽固性腹水的临床疗效确切,安全可靠,可改善患者肝肾功能,减轻临床症状。
Objective To analyze the clinical curative effect of Jianpi Lishui Formula on cirrhotic refractory ascites and its effect on liver and kidney function.Methods Ninety-two patients with cirrhotic refractory ascites according to the random number table were divided into 51 control groups and 41 research groups.The control group was treated with conventional therapy,and the research group was treated with Jianpi Lishui Formula on the basis of the control group.The clinical efficacy,abdominal circumference,body weight,24 h urine volume,abdominal B-ultrasound index,liver and kidney function,quality of life before and after treatment,and safety in both group were compared.Results The total effective rate of the research group was higher than that of the control group(P<0.05).Before treatment,abdominal circumference,body weight,24 h urine volume,abdominal B-ultrasound index,liver and kidney function,quality of life in both groups had no statistically significant differences(P>0.05).After treatment,abdominal circumference,body weight,24 h urine volume,abdominal B-ultrasound index,aspartate aminotransferase(AST),alanine aminotransferase(ALT),total bilirubin(TBIL),urea nitrogen(BUN)and serum creatinine(Cr)in both groups were decreased.The albumin(ALB)and quality of life were increased,and the changes in the above indicators of the research groups were more obvious,and there were statistical differences(P<0.05).The occurrence of adverse reactions between the two groups had no statistical difference(P>0.05).Conclusion The clinical curative effect of Jianpi Lishui Formula on refractory ascites of liver cirrhosis is accurate,safe and reliable.Jianpi Lishui Formula can improve the liver and kidney function of patients and reduce clinical symptoms.
