实时三维斑点追踪技术在中老年急性心肌梗死患者经皮冠状动脉介入治疗术后左心室整体收缩功能评估中的应用价值 被引量:19

摘要 目的探讨实时三维斑点追踪技术(3D-STI)在中老年急性心肌梗死(AMI)患者经皮冠状动脉介入治疗(PCI)术后左心室整体收缩功能评估中的应用价值.方法选取2016年7月至2018年9月我院71例行PCI术的中老年AMI患者作为研究组,纳入同期健康体检者75名作为对照组,均行超声心动图、3D-STI检查,统计2组左心室后壁厚度(LVPW)、左心室舒张末期容积(ESV)、左心室射血分数(LVEF)、左心房大小、左心室舒张末期内径(LVEDD)、整体径向应变(GRS)、整体面积应变(GAS)、整体纵向应变(GLS),并分析3D-STI左心室整体应变参数与LVEF的相关性.结果研究组ESV、左心房大小、LVEDD水平较对照组高,LVEF水平较对照组低(P<0.05),2组LVPW水平比较差异无统计学意义(P>0.05);研究组3D-STI左心室GLS、GRS、GAS水平较对照组低(P<0.05);3D-STI左心室整体应变参数GLS(r=-0.817)、GAS(r=-0.899)与LVEF呈负相关,GRS(r=0.860)与LVEF呈正相关(P<0.05).结论3D-STI能定量分析中老年AMI患者PCI术后左心室整体收缩功能,为PCI术后疗效评估提供有利依据,值得临床推广及应用. Objective To investigate the use of real-time three-dimensional speckle tracking(3D-STI)in the evaluation of left ventricular global systolic function in middle-aged and elderly patients with acute myocardial infarction(AMI)after percutaneous coronary intervention(PCI).Methods Seventy-one middle-aged and elderly patients with AMI,who underwent PCI in our hospital between July 2016 and September 2018,were included as the study group.A contemporary cohort of 75 healthy subjects were included as the control group.Both groups underwent echocardiography and 3D-STI examination.The left ventricular posterior wall thickness(LVPW),left ventricular end-diastolic volume(ESV),left ventricular ejection fraction(LVEF),left atrial size,left ventricular end-diastolic diameter(LVEDD),global radial strain(GRS),global area strain(GAS),and global longitudinal strain(GLS)were examined in the two groups.The correlation between 3D-STI left ventricular global strain parameters and LVEF was analyzed.Results The ESV,left atrial size,and LVEDD level in the study group were higher than those in the control group,and the LVEF level was lower than that in the control group(all P<0.05).There was no significant difference in the LVPW level between the two groups(P>0.05).The 3D-STI left ventricular GLS,GRS and GAS levels in the study group were lower than those in the control group(all P<0.05).The 3D-STI left ventricular global strain parameters,such as GLS(r=-0.817)and GAS(r=-0.899)were negatively correlated with LVEF,whereas the GRS(r=0.860)was positively correlated with LVEF(all P<0.05).Conclusion 3D-STI may quantitatively analyze the left ventricular global systolic function after PCI in middle-aged and elderly patients with AMI,which favors the evaluation of efficacy after PCI and justifies widespread use in the clinical settings.
