摘要
目的:探讨腹腔镜下经胆囊胆总管探查术对胆总管结石患者的临床效果。方法:选取我院就诊的78例胆总管结石患者,随机分为对照组(39例)和观察组(39例)。对照组采用腹腔镜下胆总管探查并一期缝合术进行治疗,观察组采用腹腔镜下经胆囊胆总管探查术进行治疗,比较2组手术相关指标(手术时间、术后置管时间、住院时间、术中出血量、术后引流量),免疫功能相关指标(CD3^+、CD4^+、CD8^+、CD4^+/CD8^+、IgA、IgG、IgM)及并发症。结果:与对照组比较,观察组手术时间[(92.86±10.67)vs(109.87±13.14)min]、住院时间[(8.07±1.73)vs(10.58±2.17)d]及术后置管时间[(7.16±1.58)vs(9.68±2.35)d]均较对照组短,且术中出血量[(15.43±3.68)vs(18.65±4.28)mL]及术后引流量[(27.48±4.47)vs(42.81±6.95)mL]较对照组少,差异有统计学意义(P<0.05);2组的CD3^+、CD4^+、IgA、IgG、IgM水平均较治疗前降低,且观察组高于对照组,差异有统计学意义(P<0.05);观察组并发症总发生率(2.56%)较对照组(20.51%)低,差异有统计学意义(P<0.05)。结论:采用腹腔镜下经胆囊胆总管探查术对胆总管结石患者的治疗效果显著,对免疫功能的影响较小,利于患者恢复。
Objective:To investigate the clinical effect of laparoscopic common bile duct exploration for cholecystolithiasis.Methods:78 patients with choledocholithiasis admitted to our hospital from February 2018 to April 2019 were randomly divided into a control group(39 cases)and an observation group(39 cases).The control group was treated with laparoscopic common bile duct exploration and primary suture,while the observation group was treated with laparoscopic common bile duct exploration of the gallbladder,and the clinical indicators,immune function and complications of the two groups were observed and analyzed.Results:The operation time,hospitalization time and postoperative catheterization time of the observation group were shorter than that of the control group,and the intraoperative blood loss and postoperative drainage volume were less than that of the control group,the difference was statistically significant(P<0.05).The levels of CD3^+,CD4^+,immunoglobulin A(IgA),immunoglobulin G(IgG)and immunoglobulin M(IgM)in the two groups were all lower than those before treatment,and the difference was statistically significant(P<0.05).The total incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:Laparoscopic common bile duct exploration of the gallbladder has a significant effect on the treatment of common bile duct stones,and has little effect on the immune function,which is beneficial to the recovery of patients.
