摘要
目的:探讨对重症肺炎伴呼吸衰竭患者中实施纤支镜下支气管肺泡灌洗的临床应用效果。方法:回顾性分析某院在2016年3月~2019年10月间接诊的150例重症肺炎伴呼吸衰竭患者的临床资料,按照不同治疗方式分为对照组(常规治疗+无菌吸痰管清除呼吸道异物,n=70)与观察组(常规治疗+纤支镜下支气管肺泡灌洗,n=80),对比两组血气指标及炎症反应。结果:治疗1d后两组血氧饱和度(SaO2)水平比治疗前高,动脉二氧化碳分压(PaCO2)水平比治疗前低,且观察组SaO2水平比对照组高,PaCO2水平比对照组低,差异有统计学意义(P<0.05);治疗1周后两组C反应蛋白(CRP)与正五聚体蛋白-3(PTX-3)水平比治疗前低,且观察组CRP、PTX-3水平比对照组低,差异有统计学意义(P<0.05)。结论:纤支镜下支气管肺泡灌洗治疗重症肺炎伴呼吸衰竭患者,可显著改善血气指标,减轻炎症反应。
Objective:To investigate the application effect of bronchofiberscope bronchoalveolar lavage in patients with severe pneumonia and respiratory failure.Methods:The clinical data of 150 patients with severe pneumonia and respiratory failure who admitted in a hospital from March 2016 to October 2019 were retrospectively analyzed.They were divided into control group(routine treatment+aseptic sputum suction tube to remove foreign body in respiratory tract,n=70)and observation group(routine treatment+bronchofiberscope bronchoalveolar lavage,n=80)according to different treatment methods.The blood gas indexes and inflammatory response were compared between two groups.Results:After treatment for 1d,the levels of oxygen saturation(SaO 2)in two groups were higher than those before treatment,the partial pressure of carbon dioxide(PaCO 2)was lower than that before treatment,the SaO 2 in observation group was higher than that in control group,and the PaCO 2 in observation group was lower than that in control group(P<0.05);After treatment for 1w,the C-reactive protein(CRP)and pentraxin-3(PTX-3)in two groups were lower than those before treatment,and the levels of CRP and PTX-3 in observation group were lower than those in control group(P<0.05).Conclusion:Bronchofiberscope bronchoalveolar lavage in treatment of patients with severe pneumonia and respiratory failure can improve blood gas indexes and alleviate inflammatory response.
