摘要
目的:探讨鼻腔冲洗联合中药温肺汤治疗变应性鼻炎(AR)的临床疗效及对免疫指标的影响。方法:将108例AR患者按照随机数字表法分为2组,每组54例,对照组给予鼻腔冲洗联合氯雷他定片治疗,治疗组给予鼻腔冲洗联合中药温肺汤治疗,比较两组治疗前后鼻部主要症状评分、外周血Th17细胞、调节性T细胞(Treg)和嗜酸性粒细胞(EOS)含量;并比较两组血清白细胞介素-17(IL-17)、转化生长因子-β1(TGF-β1)和免疫球蛋白E(IgE)水平,比较两组治疗总有效率。结果:治疗后治疗组鼻部症状状鼻塞、鼻痒、喷嚏、流清涕评分低于对照组(P<0.05,P<0.01);Th17细胞、L-17、IgE、EOS低于对照组(P<0.01),Treg细胞、TGF-β1高于对照组(P<0.01),治疗总有效率高于对照组(P<0.05),差异均有统计学意义。结论:鼻腔冲洗联合温肺汤口服可改善Th17/Treg细胞免疫失衡,减少IgE、EOS分泌,缓解了AR临床症状,治疗AR效果显著。
Objective:To investigate the clinical effect of nasal irrigation combined with Wenfei Decoction on allergic rhinitis(AR) and its influence on immune indexes.Methods:108 patients with AR were divided into two groups according to the random number table method,with 54 cases in each group.The controlled group were treated with nasal irrigation combined with loratadine tablets.The treatment group were treated with nasal irrigation combined with Wenfei Decoction.The nasal symptoms score,the content of Th17 cells in peripheral blood,regulatory T cells(Treg) and eosinophils(EOS) of the patients of the two groups were compared before and after treatment.The levels of interleukin-17(IL-17),transforming growth factor-beta 1(TGF-beta 1) and immunoglobulin E(IgE) were compared between the two groups.Results:After treatment,the scores of symptomatic nasal obstruction,pruritus,sneezing and runny nose of the treatment group were lower than those of the controlled group(P < 0.05,P < 0.01),Th17 cells,IL-17,IgE and EOS were lower than those of the controlled group(P < 0.01),Treg cells and TGF-beta 1 were higher than those of the controlled group(P < 0.01),and the total effective rate of treatment was higher than that of the controlled group(P < 0.05).The difference was statistically significant.Conclusion:Nasal irrigation combined with Wenfei Decoction can improve immune imbalance of Th17/Treg cells,reduce IgE and EOS secretion,alleviate the clinical symptoms of AR,and it has a significant therapeutic effect on AR.
