摘要
目的:分析新生儿败血症的相关因素,为临床诊治提供循证支持。方法:收集2013年1月-2018年1月本院620例新生儿败血症的临床资料进行回顾性分析,根据早产与足月产分为早产儿组(n=172)与足月儿组(n=448)。比较两组临床表现、常见并发症及实验室常见指标异常情况,分析病原体分布情况。结果:早产儿组青紫、皮肤花纹、腹胀、黄疸、硬肿及呼吸暂停发生率均高于足月儿组(P<0.05)。早产儿组呼吸衰竭、休克及坏死性小肠结肠炎发生率均高于足月儿组(P<0.05)。早产儿组白细胞计数(WBC)异常、血小板(PLT)降低及C反应蛋白(CRP)升高发生率均高于足月儿组(P<0.05)。早产儿以革兰阴性菌感染为主,足月儿以革兰阳性菌感染为主;早产儿真菌感染较高。结论:新生儿败血症的临床症状缺乏特异性,早产儿常合并并发症;早产儿与足月儿感染菌谱有差异;实验室检查有助于新生儿败血症的诊断,多种检测手段联合应用可提高诊断准确率。
Objective:To analyze the related factors of neonatal sepsis and provide evidence-based support for clinical diagnosis and treatment.Method:The clinical data of 620 cases of neonatal sepsis in our hospital from January 2013 to January 2018 were collected for retrospective analysis.They were divided into the premature group(n=172)and the full-term group(n=448)according to preterm and full-term births.The clinical manifestations,common complications and common laboratory abnormalities of the two groups were compared,and pathogen distribution was analyzed.Result:The incidences of cyanosis,skin pattern,abdominal distension,jaundice,swelling and apnea in the premature group were higher than those in the full-term group(P<0.05).The incidences of respiratory failure,shock and necrotizing enterocolitis in the premature group were higher than those in the full-term group(P<0.05).The incidences of abnormal white blood cell(WBC),decreased platelet(PLT)and increased C reactive protein(CRP)in the premature group were higher than those in the full-term group(P<0.05).The preterm infants were mainly infected by gram-negative bacteria,while the full-term infants were mainly infected by gram-positive bacteria.Premature infants have higher rates of fungal infection.Conclusion:The clinical symptoms of neonatal sepsis are lack of specificity.There are differences in the infection spectrum between premature and full-term infants.Laboratory examination is helpful for the diagnosis of neonatal sepsis.Combined application of multiple detection methods can improve the diagnostic accuracy.
