摘要
目的:探讨中老年女性尿失禁严重程度的相关影响因素及对生活质量的影响。方法:2018年6月至2019年6月在汕头大学医学院第一附属医院和汕头市第三人民医院的门诊、住院部及社区中采用便利抽样的方法,抽取年龄≥45岁且存在尿失禁的中老年女性315例进行问卷调查。了解其一般资料、患病情况、影响因素、生活质量情况、就医状况等。结果:中老年女性患轻度尿失禁者71.4%(225/315),压力性尿失禁66.3%(209/315)。尿失禁患者中对尿失禁知识有了解的仅2.9%,有就诊的患者26.3%。多元有序Logistic回归分析发现与尿失禁严重程度有关的因素有年龄、睡眠质量状况、月经初潮年龄、流产史、是否共病(≥2种疾病)、排尿时括约肌的控制情况、咖啡消耗情况、饮水习惯、吸烟情况、独居、绝经时长。不同类型尿失禁患者生活质量总分和社交维度得分:混合性尿失禁>压力性尿失禁>急迫性尿失禁,差异有统计学意义(P<0.05)。结论:压力性尿失禁是中老年女性最常见的尿失禁,且以轻度尿失禁常见。混合性尿失禁的生活质量低于急迫性尿失禁和压力性尿失禁。
Objective:To explore the related factors of the severity of urinary incontinence and the influence on the quality of life of middle-aged and old women.Methods:From June 2018 to June 2019,315 middleaged and old women aged≥45 with urinary incontinence were selected for questionnaire survey in outpatient department,inpatient department and community of the First Affiliated Hospital of Shantou University Medical College and the Third People’s Hospital of Shantou.To understand the general information,disease status,influencing factors,quality of life,medical status,etc.Results:71.4%(225/315)of middle-aged and old women suffered from mild urinary incontinence,and 66.3%(209/315)of them suffered from stress urinary incontinence.Only 2.9%of the patients with urinary incontinence knew the knowledge of urinary incontinence,and 26.3%of the patients with medical treatment.Multiple logistic regression analysis showed that the factors related to the severity of urinary incontinence were age,sleep quality,menarche age,abortion history,comorbidity(≥2 diseases),sphincter control during urination,coffee consumption,drinking habits,smoking,living alone and menopause duration.The total score of quality of life and social dimension score of different types of urinary incontinence:mixed urinary incontinence>stress urinary incontinence>urgency urinary incontinence,the difference was statistically significant(P<0.05).Conclusion:Stress urinary incontinence is the most common urinary incontinence in middle-aged and old women,and mild urinary incontinence is common.The quality of life of mixed incontinence was lower than that of urgent incontinence and stress incontinence.
