气压治疗仪对剖宫产后下肢静脉血流动力学及深静脉血栓形成发生率的影响研究 被引量:70

Study on the effects of barometric therapeutic apparatus for hemodynamics of lower extremity vein and incidence rate of DVT post cesarean section

摘要 目的:分析气压治疗仪对剖宫产后下肢静脉血流动力学及深静脉血栓形成(DVT)发生率的影响。方法:选取在医院接受剖宫产手术的120例产妇,按随机数表法将其分为观察组和对照组,每组60例。对照组剖宫产术后按常规临床路径进行干预,观察组在常规干预的基础上配合使用气压治疗仪,比较两组一般手术情况、术后凝血功能、下肢静脉血流动力学指标及DVT发生率。结果:两组手术时间、术中出血量和术后卧床时间比较差异无统计学意义。术后3 d观察组产妇下肢血流峰速度、血流平均速度和血流量显著高于对照组,其差异有统计学意义(t=10.511,t=10.851,t=7.878;P<0.05);对照组产妇红细胞聚集指数、D-二聚体和纤维蛋白原(Fib)较术前均显著上升,其差异有统计学意义(t=-15.491,t=-46.299,t=-9.987;P<0.05),观察组红细胞聚集指数、Fib较术前均未见显著变化,D-二聚体显著较术前显著上升,但仍显著低于对照组,两组比较差异有统计学意义(t=9.303,t=12.503,t=4.611;P<0.05);观察组产妇下肢肿胀、下肢疼痛及DVT发生率则显著低于对照组,差异有统计学意义(χ^2=4.904,χ^2=5.175,χ^2=4.324;P<0.05)。结论:剖宫产术后应用气压治疗仪可有效改善下肢血流动力学,降低DVT风险,发挥DVT预防效应,具有一定临床意义。 Objective:To analyze the effects of barometric therapeutic apparatus on hemodynamics of lower extremity vein and incidence rate of deep vein thrombosis(DVT)post cesarean section.Methods:120 puerpera who underwent cesarean section in hospital were divided into observation group and control group according to the random number table method,with 60 cases in each group.Control group was intervened as routine clinical pathway post cesarean section,and observation group used barometric therapeutic apparatus on the routine intervention.The general surgical conditions,postoperative coagulation function,hemodynamic indicators of lower extremity vein and incidence rate of DVT between the two groups were compared.Results:There were no significant differences in operative time,intraoperative blood loss and postoperative bed rest time between the two groups.However,the peak velocity,average velocity and volume of blood flow of lower extremity at 3d post operation of observation group were significantly higher than those of control group(t=10.511,t=10.851,t=7.878,P<0.05).In control group,the erythrocyte aggregation index,D-dimer and fibrinogen(Fib)at 3d post operation were significantly higher than those pre operation(t=-15.491,t=-46.299,t=-9.987,P<0.05).In observation group,there were no significant changes in the erythrocyte aggregation index and Fib between pre and post operation except D-dimer,which still was significantly lower than that of control group(t=9.303,t=12.503,t=4.611,P<0.05).The incidence rates of lower extremity swelling,lower extremity pain and DVT of observation group were significantly lower than those of control group(t=4.904,t=5.175,t=4.324,P<0.05),respectively.Conclusion:The application of barometric therapeutic apparatus post cesarean section can effectively improve the hemodynamics of lower extremities,and reduce the risk of DVT,and exert the preventive effect for DVT.Thus it has a certain clinical significance.
