老年糖尿病患者黄斑区视网膜厚度与糖化血红蛋白的关系 被引量:1

Relationship between macular retinal thickness and glycosylated hemoglobin in elderly patients with diabetes

摘要 目的探讨老年早期糖尿病患者黄斑区视网膜形态学变化特点,并进一步研究视网膜厚度与生化指标糖化血红蛋白(Hb A1c)的关系。方法选取2018年1月~2019年6月在我院内分泌科被确诊为1型或2型糖尿病患者40例(40眼),所选患者均接受胰岛素或降糖药物治疗;对照组40例(40眼)均为我院健康体检者;两组患者在年龄和性别上分布一致。应用光学相干断层扫描(OCT)测量糖尿病组和对照组患者的黄斑区视网膜厚度,应用生化仪检测两组患者的HbA1c。比较两组患者黄斑区视网膜的厚度,包括黄斑区平均视网膜厚度和黄斑容积,并分析糖尿病组黄斑区厚度与相关生化指标HbA1c的关系。结果糖尿病组患者黄斑区平均视网膜厚度厚于对照组(t=6.877,P<0.01),黄斑容积大于对照组(t=5.832,P<0.01),黄斑区视网膜厚度与HbA1c比呈正相关(r=0.589,P<0.01)。结论检测糖化血红蛋白(HbA1c)的变化,有利于预测老年早期糖尿病视网膜病变并发症的发生。 Objective To explore the morphological changes of the retina in the macular area of early diabetic patients,and to further study the relationship between retinal thickness and biochemical index glycosylated hemoglobin(HbA1 c).Methods A total of 40 patients(40 eyes) with type 1 or type 2 diabetes diagnosed in our department of endocrinology from January 2018 to June 2019 were selected. All patients were treated with insulin or hypoglycemic agents. 40 cases of healthy subjects(40 eyes) in our hospital were selected as control group. The two groups were consistent in age and gender. The macular retinal thickness was measured by optical coherence tomography(OCT) in diabetic and control groups. And the HbA1 c level of two groups was detected by biochemical analyzer. The thickness of the macular retina was compared between the two groups, including the average retinal thickness in the macular area and macular volume.And the relationship between the thickness of the macular area in the diabetic group and the related biochemical index HbA1 c was analyzed. Results The average retinal thickness in the macular area of the diabetic group was thicker than that of the control group(t=6.877, P<0.01), and the macular volume was larger than that of the control group(t=5.832,P<0.01). The retinal thickness in the macular area was positively correlated with the HbA1 c ratio(r=0.589, P<0.01).Conclusion Detection of changes in glycosylated hemoglobin(HbA1 c) is conducive to predict the occurrence of complications of early diabetic retinopathy.
