ICU肠内营养患者发生非糖尿病性高血糖的影响因素研究 被引量:2

摘要 目的分析ICU行肠内营养(EN)非糖尿病性患者发生高血糖的影响因素。方法收集187例接受EN治疗的非糖尿病性危重患者的相关资料,采用自设问卷调查相关影响因素。结果 42(22.46%)例患者伴发高血糖。APACHE II评分、鼻饲速度、EN液类型、EN输注途径、肠外营养、抗生素使用、机械通气和禁食为ICU非糖尿病性EN患者发生高血糖的独立危险因素(P<0.05)。结论积极识别ICU非糖尿病性EN患者发生高血糖的独立危险因素,将有助于针对性修正EN方案,提高临床治疗效果。 Objective To investigate the influencing factors of non-diabetic hyperglycemia in ICU patients receiving enteral nutrition(EN). Methods A questionnaire survey was conducted to investigate the current situation and related risk factors of 187 cases of non-diabetic hyperglycemia critical patients receiving EN were analyzed. Results Of 187 non-diabetic patients receiving EN treatment, 42 patients(22.46%)were accompanied with hyperglycemia. APACHE Ⅱ scores, nasal feeding speed, EN fluid type, EN infusion route, parenteral nutrition, antibiotic use, mechanical ventilation, fasting were statistically significant in patients with hyperglycemia group and non-hyperglycemia group(P<0.05), and were independent risk factors for hyperglycemia in ICU patients receiving EN. Conclusion Effective identification of independent risk factors for hyperglycemia in non-diabetic patients receiving EN is helpful to the revision of EN program and the improvement of clinical treatment effect.
