摘要
目的探讨社区高血压患者治疗依从性的影响因素,并分析治疗依从性和治疗态度与信念的相关性。方法采用一般资料调查表、高血压治疗依从性量表(TASHP)、高血压治疗态度与信念量表(ATRABS)对社区卫生服务中心236例高血压患者进行调查。结果社区高血压患者治疗依从性与年龄、月收入、医疗费用支付方式以及高血压病程有关(P<0.05)。社区高血压患者ATRABS总分(70.41±9.01)分,属于中下水平。社区高血压患者的治疗依从性总分(91.68±11.43)分,处于中等水平。社区高血压患者的治疗依从性和治疗态度与信念呈正相关(r=0.561,P<0.05)。坚持治疗信念与态度、服药治疗信念与态度、生活方式的信念与态度、月收入、治疗困扰信念与态度均是影响社区高血压患者治疗依从性的相关因素。结论社区高血压患者治疗依从性与治疗信念与态度密切相关。社区护理人员应重点对社区高血压患者治疗态度与信念进行干预,帮助患者纠正错误的、消极的态度与信念,提高治疗依从性,有效控制血压,提高生活质量。
Objective To explore the influencing factors of therapeutic adherence in community-dwelling patients with hypertension, and analyze the relation between therapeutic adherence and therapy-related attitudes and beliefs among them. Methods A total of 236 hypertensive patients in the community health service center were surveyed using the General Information Questionnaire, Therapeutic Adherence Scale for Hypertensive Patients(TASHP), Antihypertensive Therapy-Related Attitudes and Beliefs Scale(ATRABS). Results There therapeutic adherence of community-dwelling patients was influenced by age, monthly income, payment method of medical cost and course of hypertension(P<0.05). The mean overall score of ATRABS was 70.41±9.01, indicating a relatively low level of therapy-related awareness. The mean overall score of TASHP was(91.68±11.43), indicating moderate level of therapeutic adherence. Therapy-related attitudes and beliefs was positively correlated with therapeutic adherence(r=0.561,P<0.05). Entering the treatment adherence regression equation, there are five variables: monthly income, adherence to treatment attitudes and beliefs, medication attitudes and beliefs, therapeutic lifestyle attitudes and beliefs, and persistent treatment distress concerns. Conclusion The therapeutic adherence in community-dwelling hypertensive patients is closely related to their treatment beliefs and attitudes. Community healthcare workers should focus on the treatment attitudes and beliefs among community-dwelling patients with hypertension, help patients correct wrong and negative attitudes and beliefs, improve therapeutic adherence, effectively control blood pressure, and improve quality of life.
