还原型谷胱甘肽对化疗相关性肾损伤患者肾损伤标志物的影响及意义 被引量:4

Effect and significance of reduced glutathione on renal injury markers in patients with chemotherapy-related renal injury

摘要 目的探讨还原型谷胱甘肽对化疗相关性肾损伤患者肾损伤标志物的影响及其临床意义。方法将2015年10月-2018年10月本院收治的146例化疗相关性肾损伤患者纳入研究对象,将其按照随机数字表法分为研究组和对照组,各73例,对照组给予常规治疗,研究组在常规治疗基础上给予还原型谷胱甘肽,2组均连续治疗30 d。比较治疗前后2组患者肾损伤标志物[尿肾损伤分子1(Kim-1)、钠/氢交换蛋白3(NHE3)、人中性粒细胞明胶酶相关脂质运载蛋白(NGAL)、人纤溶酶原激活物抑制因子1(PAI-1)及血清胱抑素-C(Cys-C)]水平及肾功能指标[尿微量白蛋白(M-ALB)、24 h尿蛋白定量(24 h Upro)、血清β2-微球蛋白(β2-MG)、肌酐(Scr)、尿素氮(BUN)]水平。结果治疗前2组患者尿Kim-1、NGAL、NHE3、PAI-1、血清Cys-C、尿M-ALB、24 h Upro及血清β2-MG、Scr、BUN水平相比差异均无统计学意义(P>0.05),治疗后2组患者尿Kim-1、NGAL、NHE3、PAI-1、血清Cys-C、尿M-ALB、24 h Upro及血清β2-MG、Scr、BUN水平均显著低于治疗前(P<0.05),研究组患者尿Kim-1、NGAL、NHE3、PAI-1、血清Cys-C、尿M-ALB、24 h Upro及血清β2-MG、Scr、BUN水平均显著低于对照组(P<0.05)。结论还原型谷胱甘肽可明显降低化疗相关性肾损伤患者肾损伤标志物水平,并可有效促进肾功能恢复。 Objective To investigate the effect of reduced glutathione on renal injury markers in patients with chemotherapy-related renal injury and its clinical significance.Methods A total of 146 patients with chemotherapy-related renal injury admitted to our hospital from October 2015 to October 2018 were included in the study.They were divided into study group and control group according to random number table method,with 73 cases in each group.The control group was given routine treatment,while the study group was given reduced glutathione on the basis of routine treatment.Both groups were treated continuously for 30 days.The levels of renal injury markers[urine kidney injury molecule 1(Kim-1),sodium/hydrogen exchange protein 3(NHE3),human neutrophil gelatinase-associated lipocalin(NGAL),human plasminogen activator inhibitor 1(PAI-1)and serum cystatin-C(Cys-C)]and renal function indexes[urine microalbumin(urine M-ALB),24 h urine protein quantification(24 h Upro),serum β2-microglobulin(β2-MG),creatinine(Scr),urea nitrogen(BUN)]were compared between the two groups before and after treatment.Results There was no statistical significance on the differences in urinary Kim-1,NGAL,NHE3,PAI-1,serum Cys-C,urinary M-ALB,24 h Upro and serum β2-MG,Scr and BUN levels between the two groups before treatment(P>0.05).After treatment,the levels of Kim-1,NGAL,NHE3,PAI-1,Cys-C,M-ALB,24 h Upro and serum β2-MG,Scr and BUN in the two groups were significantly lower than those before treatment(P<0.05).The levels of Kim-1,NGAL,NHE3,PAI-1,Cys-C,M-ALB,24 h Upro and serumβ2-MG,Scr and BUN in the study group were significantly lower than those in the control group(P<0.05).Conclusion Reduced glutathione can significantly reduce the level of renal injury markers in patients with chemotherapy-related renal injury,and can effectively promote the recovery of renal function.
