摘要
目的探讨强化健康管理方式对老年功能性便秘(functional constipation,FC)患者便秘症状、生活质量及精神状态的影响。方法将197例老年FC患者分为两组,药物组给予常规通便药物治疗,管理组在此基础上,分别于治疗前、1个月、2个月以强化管理方式进行便秘知识宣教、饮食与运动指导,记录并比较治疗前、2个月、6个月时两组患者便秘症状、生活质量、精神状态各量表评分的差异。结果①两组患者基线资料差异无统计学意义(P>0.05)。②便秘患者症状自评(PAC-SYM)量表评分:治疗前两组差异无统计学意义,治疗后评分均显著降低;2个月后管理组评分明显低于药物组,差异具有统计学意义;6个月后差异不明显。③便秘患者生活质量(PAC-QOL)量表评分:与便秘症状评分呈中度正相关。治疗前两组差异无统计学意义,治疗后评分均显著降低;2个月后管理组评分明显低于药物组,差异具有统计学意义;6个月后差异不明显。④精神量表(SAS/SDS)评分:与便秘症状评分呈较弱程度正相关。治疗前两组差异无统计学意义,治疗后评分均显著降低;2个月后管理组评分低于药物组,但差异无统计学意义;6个月后管理组评分仍低于药物组,差异具有统计学意义。结论强化健康管理可明显改善老年FC患者的便秘症状和生活质量,对精神心理状况的短期影响不明显,但停止管理后,患者焦虑、抑郁的精神状态仍能得到持续改善。
Objective To explore the effects of enhanced health management on the constipation symptoms,quality of life,and mental state of the elderly patients with functional constipation.Methods A total of 197 senile patients with functional constipation were divided into two groups:the control group with drug of laxatives and the management group with laxatives and enhanced health management.The patients in management group accepted healthy education or instruction once a month,totally 3 times.The change of constipation symptoms,quality of life,and mental state were observed and compared between the two groups.Results①The baseline data were comparable between the two groups(P>0.05).②The PAC-SYM Scale scores were similar at the beginning,while the scores of two groups were both decreased and the decrease extent in the management groups was larger than the control group after 2 months.While,the scores decreased similarly between the two group after 6 months.③The PAC-QOL Scale scores changed as same as PAC-SYM scores.④The SAS/SDS scores of management group were significantly lower than the control group.Conclusion Enhanced health management has good effects on elderly functional constipation by relieving constipation symptoms and improving the patients’quality of life in the short term,and it also has long-term effects on the patients’mental state.
