摘要
目的初步探讨结核分枝杆菌(MTB) ATP结合盒外排泵基因Rv1217c、Rv1218c的表达与不同耐药表型的关系,为耐药结核病的有效防控提供科学依据。方法选择宁波市疾病预防控制中心收集的27例耐多药结核分枝杆菌临床分离株(MDR-TB)、34例耐药且非耐多药结核分枝杆菌及15例全敏感结核分枝杆菌为研究对象。提取菌株RNA,应用RT-PCR方法检测ATP结合盒外排泵基因Rv1217c^Rv1218c的相对表达量,采用t检验分析外排泵基因表达量在不同耐药表型菌株中的差异。结果与全敏感组(0. 51±0. 30)比较,Rv1217c基因表达量在耐利福平组(2. 12±1. 68)、耐异烟肼组(1. 92±1. 69)、耐链霉素组(1. 99±1. 79)及耐乙胺丁醇组(3. 18±2. 06)均升高,差异均有统计学意义(t值分别为5. 81、5. 33、4. 82、5. 14,P <0. 01)。与全敏感组(0. 73±0. 28)比较,Rv1218c基因表达量在耐利福平组(2. 63±1. 66)、耐异烟肼组(2. 51±1. 68)、耐链霉素组(2. 39±1. 64)及耐乙胺丁醇组(3. 75±1. 62)均升高,差异均有统计学意义(t值分别为6. 98、6. 91、5. 79、7. 35,P <0. 01)。耐多药组Rv1217c、Rv1218c基因表达量为(2. 59±1. 87、3. 24±1. 65),高于耐药非耐多药组(1. 01±0. 49、1. 33±0. 81),差异有统计学意义(t值分别为4. 29、5. 48,P <0. 01)。结论结核分枝杆菌ATP结合盒外排泵基因Rv1217c、Rv1218c的表达与多种药物的耐药有关。
Objective To explore the association between ATP-binding cassette efflux pump gene Rv1217c-Rv1218c expression and phenotypic drug resistance of Mycobacterium tuberculosis. Methods A total of 27 multiple drug resistant Mycobacterium tuberculosis clinical isolates,34 poly-drug resistant isolates and 15 sensitive isolates were detected. The RNA of isolated strains was extracted and then reverse transcribed. Gene expression was performed by real-time PCR and data was analysed by t test analysis. Results The expression of Rv1217c in rifampicin-resistant group( 2. 12 ± 1. 68),isoniazid-resistant group( 1. 92 ± 1. 69),streptomycin-resistant group( 1. 99 ± 1. 79) and ethambutol-resistant group( 3. 18 ± 2. 06) were all higher than sensitive isolates( 0. 51 ± 0. 30),with the differences statistically significant( t = 5. 81,t = 5. 33,t = 4. 82,t = 5. 14,P < 0. 01). The expressions of Rv1218c in rifampicin-resistant group( 2. 63 ± 1. 66),isoniazid-resistant group( 2. 51 ±1. 68),streptomycin-resistant group( 2. 39 ± 1. 64) and ethambutol-resistant group( 3. 75 ± 1. 62) were all higher than sensitive isolates( 0. 73 ± 0. 28),with the differences statistically significant( t = 6. 98,t = 6. 91,t = 5. 79,t = 7. 35,P < 0. 01).The expression of Rv1217c-Rv1218c in multidrug resistant group were( 2. 59 ± 1. 87,3. 24 ± 1. 65),which were both higher than poly-drug resistant group,with the differences statistically significant( t = 4. 29,t = 5. 48,P < 0. 01). Conclusion The expression of Rv1217c-Rv1218c is associated with multiple drug resistance of Mycobacterium tuberculosis.
