摘要
目的探讨品管圈(quality control circle,QCC)在降低门诊处方不合格率的实践效果。方法在西宁市第一人民医院门诊药房成立QCC,收集2017年1~12月门诊西药房处方286774张,其中不合格处方14176张(4.94%)。采用PDCA循环拟定计划,帕累托图分析不合格处方的主要原因,设定2.47%为不合格率改善的目标,制定对策并实施,观察实施效果。结果2018年1~12月共开具处方253256张,发生不合格处方5048张,处方不合格率1.99%,目标达成率119.4%。QCC活动后,圈员自评团队协作、责任心、沟通配合能力、执行力、愉悦感、自信心、解决问题能力和品管手法均得到提升。结论QCC活动能够有效降低门诊处方不合格率,有利于促进临床合理用药,提高药学服务质量。
Objective To investigate the the practical effects of quality control circle(QCC)in reducing the unqualified rate of outpatient prescriptions.Methods QCC was established in our hospital from January 2017 to December 2017,and 286,774 prescriptions were collected in outpatient pharmacy,of which,14,176 prescriptions(4.94%)were unqualified.PDCA cycle was used to draw up plans.Pareto diagram was used to analyze the main causes of unqualified prescriptions.The unqualified rate 2.47%was taken as the improving target,and the countermeasures were formulated and implemented,and the implementation effects were observed.Results A total of 253,256 outpatient prescriptions were prescribed in our hospital from January 2018 to December 2018,in which,5048 outpatient prescriptions were unqualified,with the unqualified rate being 1.99%,and the target achievement rate was 119.4%.After implementation of QCC activity,the team cooperation,responsibility,communication and cooperation ability,execution ability,pleasure feeling,self-confidence,problem solving ability and quality control techniques were improved.Conclusion QCC activities can effectively reduce the unqualified rate of outpatient prescriptions,promote clinical rational drug use and improve the quality of pharmaceutical care.
