摘要
目的分析乳腺纤维导管镜下地塞米松、甲硝唑、庆大霉素灌洗治疗非哺乳期乳腺炎的疗效。方法选取2011年1月至2016年1月收治的非哺乳期乳腺炎患者共243例,按治疗方法不同分为治疗组113例和对照组110例。2组均给予常规抗炎治疗,头孢呋辛钠2.0 g,2次/d,静脉滴注,100 ml 0.9%氯化钠溶液+地塞米松10 mg至2.5 mg计量递减,1次/d,静脉滴注,疗程10 d。治疗组在此基础上给予乳腺纤维导管镜冲洗疗法,乳腺纤维导管镜下病变处乳管地塞米松、甲硝唑、庆大霉素混合液灌洗治疗。比较2组疗效,并对相关病理参数进行分析。结果治疗组总有效率为77.8%高于对照组的64.5%,差异有统计学意义(P<0.05)。纤维乳管镜冲洗疗法在肿块型非哺乳期乳腺炎有效率最高,在瘘管型非哺乳期乳腺炎中最低;肿块型的有效率与月经情况及发病时间有相关性(P<0.05),其中与发病时间成负相关(r=-0.277,P=0.029),发病时间越短,治愈率越高。结论乳腺纤维导管镜冲洗疗法在早期肿块型非哺乳期乳腺炎中治疗效果显著。
Objective To investigate the therapeutic effects of fiberopticductoscopy lavage with mixed liquor of dexamethasone,metronidazole and gentamicin on non-lactation mastitis.Methods A total of 113 patients with non-lactation mastitis who were treated in our hospital from January 2011 to January 2016 were enrolled in the study,who were treated by fiberopticductoscopy lavage with mixed liquor of dexamethasone,metronidazole and gentamicin.After treatment the therapeutic effects and relevant pathological parameters were observed and compared between treatment group and control group.Results In treatment group,43 patients(38.1%)were cured,45 patients were improved and 25 patients were not cured,with total effective rate being 77.8%.In control group,30 patients were cured,41 patients were improved and 39 patients were not cured,with total effective rate being 64.5%.The results showed that the total effective rate in treatment group was higher than that in control group.Fiberopticductoscopy lavage treatment had the highest efficiency in mass-type non-lactation mastitis,and had the lowest efficiency in fistula-type non-lactation mastitis.The effective rate of mass-type non-lactation mastitis was closely related to menstrual condition and disease duration(P<0.05),which was negatively correlated with the disease duration,and the shorter the disease onset time,the higher the effective rate.The curative effects in non-menopause patients were better than those of post-menopausal patients.Conclusion Fiberopticductoscopy lavage treatment has significant curative effects on early mass-type non-lactation mastitis.
