摘要
临床资料 患者,女,64 岁,首诊以"左侧髋关节疼痛不适伴活动受限 20 天"入院.患者 2 个月前轻微扭伤出现右侧髋关节疼痛不适,20 天前出现左侧髋关节疼痛不适伴活动明显受限,疼痛剧烈,无法站立行走.无下肢放射性疼痛,无晨僵、低热盗汗、胸闷憋气等表现.入院后,根据查体及影像学所见初步诊断为:双侧股骨颈骨折.患者既往无高血压、糖尿病及心脑血管疾病病史,否认家族中有遗传倾向性疾病及传染性疾病.查体:臀部皮肤无破损,无红肿,皮温略高,双侧髋关节外旋 60° 畸形,双侧髂脊不对称,双下肢纵向叩击痛阳性,大转子区压痛明显.双下肢相关的检查及肌力因患者疼痛无法配合检查.余肢未见明显异常.完善入院的相关检查,心电图、心脏超声、胸片及下肢血管超声未见明显异常.
This article reports a case of osteoporosis and pathological fracture of the femoral neck following hyperparathyroidism. A 64-year-old female complained right hip pain and discomfort two months ago. Left hip pain with activity limit was observed 20 days ago. Pathological fracture caused by hyperparathyroidism was finally determined by clinical manifestation, surgical examination, biochemical indicator and imaging data. The report aims to provide references for its treatment.
