摘要
目的探究非小细胞肺癌(NSCLC)合并下呼吸道感染患者病原菌分布及与病情进展相关性。方法选取2015年6月至2018年8月延安大学附属医院肿瘤内科收治的NSCLC患者126例。采集NSCLC患者痰液进行细菌培养及鉴定。观察不同种属病原菌在NSCLC并发下呼吸道感染患者中的分布情况。结果 NSCLC并发下呼吸道感染患者中革兰阴性菌检出率为61.18%,革兰阳性菌检出率为24.71%,真菌检出率为14.12%。革兰阴性菌中大肠埃希菌检出率为40.38%,肺炎克雷伯菌检出率为32.69%,铜绿假单胞菌检出率为13.46%,鲍曼不动杆菌检出率为7.69%,其他细菌检出率为5.77%。革兰阳性菌中溶血葡萄球菌检出率为42.86%,金黄色葡萄球菌检出率为38.10%,肺炎链球菌检出率为9.52%,粪肠球菌检出率为9.52%。真菌中白假丝酵母检出率为58.33%,热带假丝酵母检出率为25.00%,光滑假丝酵母检出率为16.67%。年龄≥60岁、吸烟、TNM分期III^IV期、营养不良、住院时间≥14 d的NSCLC合并下呼吸道感染患者病原菌检出率均高于年龄<60岁、不吸烟、I^II期、营养良好、住院时间<14 d的患者(均P<0.05)。结论革兰阴性菌更易感染NSCLC合并下呼吸道感染患者。患者TNM分期、年龄等病情进展因素与病原菌感染率具有相关性。
Objective To investigate the distribution of pathogenic bacteria in non-small cell lung cancer(NSCLC) complicated with lower respiratory tract infection and its correlation with disease progression. Methods The sputum specimens of 126 NSCLC patients in Yanan University hospital from June 2015 to August 2018 were collected for bacterial culture and identification. Then the distribution of pathogenic bacteria of different genera in patients with NSCLC complicated with lower respiratory tract infection was observed. Results The detection rates of Gram-negative bacteria, Gram-positive bacteria and fungi were 61.18%, 24.71% and 14.12% respectively in NSCLC patients with lower respiratory tract infection. The detection rates of E. coli, Klebsiella pneumoniae, Pseudomonas aeruginosa, Acinetobacter baumannii and other bacteria were 40.38%, 32.69%, 13.46%, 7.69% and 5.77% respectively. The detection rates of Staphylococcus hemolyticus, Staphylococcus aureus, Streptococcus pneumoniae and Enterococcus faecalis were 42.86%, 38.10%, 9.52% and 9.52% respectively. The detection rates of Candida albicans, Candida tropicalis and Candida glabrata were 58.33%, 25.00% and 16.67% respectively. Patients with age ≥60, smoking, TNM stage III-IV, malnutrition, and hospital stay >14 days had higher infection rate than those with age <60, nonsmoking, stage I-II, good nutrition, and hospital stay <14 days(all P<0.05). Conclusion In patients with NSCLC complicated with lower respiratory tract infection, the infection rate of Gram-negative bacteria is relatively high, and the progression factors such as TNM stage and age are positively correlated with the infection rate of pathogens.
