摘要
目的:研究四妙勇安汤加减联合免疫抑制剂及醋酸泼尼松片对轻中度活动期狼疮性肾炎(LN)患者病情程度(SLEDAI积分)及血沉(ESR)、补体C3(C3)水平的影响。方法:选取我院78例LN患者,根据治疗方案不同分为对照组(n=39)和研究组(n=39)。对照组采用免疫抑制剂(环磷酰胺)及醋酸泼尼松片治疗,研究组于对照组基础上联合四妙勇安汤加减治疗。对比两组治疗效果、不良反应发生情况、治疗前后SLEDAI积分、中医证候积分、ESR、C3水平、肾功能指标[24 h尿蛋白定量、尿素氮(BUN)、肌酐(Scr)]。结果:研究组治疗总有效率(92.31%)高于对照组(74.36%)(P<0.05)。疗程结束后:两组SLEDAI、中医证候积分低于治疗前,且研究组低于对照组(P<0.05);两组ESR水平低于治疗前,且研究组低于对照组,C3水平高于治疗前,且研究组高于对照组(P<0.05);两组24 h尿蛋白定量、BUN、Scr低于治疗前,且研究组低于对照组(P<0.05)。研究组不良反应总发生率(15.38%)与对照组(20.51%)无明显差异(P>0.05)。结论:采用四妙勇安汤加减联合免疫抑制剂及醋酸泼尼松片轻中度活动期LN患者的疗效显著,能减轻患者病情程度,缓解临床症状,调节ESR、C3水平,提高肾功能,且具有一定安全性。
Objective:To study the effects of Simiao Yong’an decoction combined with immuno suppressive agents and prednisone acetate on the severity of disease (SLEDAI score),erythrocyte sedimentation rate (ESR) and complement C3 (C3) in patients with mild to moderate active lupus nephritis (LN).Methods:78 patients with LN in our hospital were selected and divided into control group (n=39) and study group (n=39) according to different treatment schemes.The control group was treated with immuno suppressive agents (cyclophosphamide) and prednisone acetate tablets.The study group was combined with Simiao Yong’an decoction on the basis of the control group.The treatment effects,adverse reactions,SLEDAI points before and after treatment,TCM syndrome points,ESR,C3 levels,and renal function indicators were compared between the two groups [24-hour urinary protein quantification,urea nitrogen (BUN),and creatinine (Scr)].Results:The total effective rate of treatment in the study group (92.31%) was higher than that in the control group (74.36%) (P<0.05).After the course of treatment:the SLEDAI and TCM syndrome scores of the two groups were lower than before the treatment,and the study group was lower than the control group (P<0.05).The ESR levels of the two groups were lower than before treatment,and the study group was lower than the control group.The C3 level was higher than before treatment,and the research group was higher than the control group (P<0.05).The 24 h urinary protein quantification,BUN,and Scr in the two groups were lower than before treatment,and the study group was lower than the control group (P<0.05).The total incidence of adverse reactions in the study group (15.38%) was not significantly different from the control group (20.51%) (P>0.05).Conclusion:The use of Simiao Yong’an decoction combined with immuno suppressive agents and prednisone acetate in mild to moderately active LN patients has significant curative effects,can reduce the patient’s condition,relieve clinical symptoms,regulate ESR and C3 levels,and improve renal function.And it has some security.
