摘要
目的:观察身痛逐瘀汤对手部骨折患者的疗效及手部肿胀消退时间、手功能恢复的影响。方法:选取平顶山市第一人民医院2016年7月-2018年6月收治的手部骨折患者76例,根据治疗方法不同分为2组。对照组患者实施微型钢板治疗,术后采用常规功能锻炼,观察组在对照组基础上口服身痛逐瘀汤治疗,分析2组患者治疗后的临床疗效。结果:2组全血黏度(高切、低切)、血浆黏度、红细胞压积均较治疗前降低(P<0.05);血清胰岛素样生长因子-Ⅰ(IGF-Ⅰ)、骨形态发生蛋白-2(BMP-2)水平均较治疗前升高(P<0.05)。治疗后与对照组比较,观察组手部消肿时间、住院时间、愈合时间均较短(P<0.05),全血黏度(高切、低切)、血浆黏度降低(P<0.05),IGF-Ⅰ、BMP-2水平升高(P<0.05),红细胞压积差异无统计学意义(P>0.05)。观察组优良率为94.73%,对照组为78.95%,差异有统计学意义(P<0.05)。结论:手部骨折患者服用身痛逐瘀汤可改善患者血液流变学指标,提高IGF-Ⅰ、BMP-2的表达水平,促进肿胀消退,骨骼愈合及手功能恢复。
Objective:To observe the curative effect of Shentong Zhuyu tang for patients with hand fracture and the effect on the regression time of swelling of the hands and the recovery of hand function. Methods:Selected 76 cases of patients with hand fracture enrolled in Pingding shan First People’s Hospital from July in 2016 to June in 2018 and divided them into two groups according to different treatment methods. Both groups were treated with mini-plate and adopted routine functional exercise after operation and the observation group was additionally given the oral administration of Shentong Zhuyu tang. Analyzed the clinical effect after treatment in the two groups. Results:The whole blood viscosity(in high shear rate and low shear rate), plasma viscosity and hematocrit in the two groups were all decreased when compared with those before treatment(P < 0.05);the levels of Serum insulin-like growth factor-I(IGF-I);Bone morphogenetic protein-2(BMP-2) were increased when compared with those before treatment(P < 0.05). Compared with the control group after treatment, the detumescence time of swelling of the hands,hospitalization time and healing time in the observation group were shorter(P <0.05),the whole blood viscosity(in high shear rate and low shear rate) and plasma viscosity were decreased(P < 0.05),the levels of IGF-I and BMP-2 were increased(P < 0.05) and there was no significant difference being found in the comparison of the hematocrit(P > 0.05). The excellent and good rate was 94.73% in the observation group and 78.95% in the control group,difference being significant(P < 0.05). Conclusion:The application of Shentong Zhuyu tang for patients with hand fracture can improve the indexes of hemorheology in patients, enhance the expression levels of IGF-I and BMP-2 and promote the reduction of swelling,the healing of the bone and the recovery of hand function.
