摘要
目的探讨在单纯冠状动脉CT血管造影(CTA)对比剂用量下,256排Revolution CT行胸痛三联CTA的成像质量与强化效果。方法搜集30例因急性胸痛入院,使用256排Revolution CT在65 ml对比剂剂量下行胸痛三联CTA检查的患者为研究组,选取同时期、相同对比剂剂量下使用Revolution CT行单纯冠状动脉CTA检查的30例患者为对照组。测量两组右冠状动脉、左主干、左前降支、左回旋支的CT值、噪声,比较两组的信噪比(SNR)、对比噪声比(CNR)。将冠状动脉分为4个等级进行主观评价,评价冠状动脉各节段得分的差异,肺动脉及胸主动脉以CT值>200 HU作为满足诊断标准。两组计量资料的比较采用t检验或非参数秩和检验,等级资料的比较采用非参数秩和检验,P<0.05表示差异具有统计学意义。结果研究组与对照组右冠状动脉、左主干、左前降支、左回旋支的CT值(t/z=0.463、0.576、0.148、0.962,P=0.646、0.574、0.882、0.34,P均>0.05)、噪声(t/z=0.699、-1.309、-1.834、-0.702,P=0.487、0.190、0.067、0.486,P>0.05)、信噪比(t/z=-1.131、-0.858、1.941、0.098,P=0.258、0.391、0.057、0.328,P均>0.05)及右冠状动脉的CNR(t=0.531,P=0.598),无显著差异,研究组与对照组冠状动脉各节段主观评分无显著差异(P均>0.05),研究组肺动脉主干CT值为(558±85) HU,CT值均>200 HU,胸主动脉CT值为(486±110) HU,CT值均>200 HU。结论 256排Revolution CT在与单纯冠状动脉CTA相同对比剂剂量下行胸痛三联CTA检查,不仅可获得相同的冠状动脉图像,而且可获得良好的肺动脉及胸主动脉图像。
Objective The aim of study was to evaluate image quality and arterial enhancement of triple-rule-out(TRO) against a dedicated coronary CT angiography(CTA) protocols when using the same amount of contrast medium. Methods Sixty patients who underwent TRO(n=30) or CTA(n=30) protocols were enrolled in this study. Total contrast medium(65 ml) was kept constant. All scans were performed using a 256-row CT scanner(Revolution CT,GE Healthcare). The arterial enhancement and noise in right coronary artery( RCA),left main artery( LMA),left anterior descending( LAD) and left circumflex( LCX) artery were recorded. Signal-to-noise( SNR) and contrast-to-noise( CNR) ratio were calculated. Subjective image quality was evaluated on a four-point scale. A minimum arterial enhancement of 200 HU is required for the diagnosis of pulmonary trunk and ascending thoracic aorta. The student t-test( normal distributed value) and Mann-Whitney U test( non-normal distributed value) were used for qualitative and quantitative assessment,respectively. P < 0. 05 indicates the result is statistically significant. Results There was no significant difference in arterial enhancement( t/z = 0. 463,0. 576,0. 148,0. 962,P = 0. 646,0. 574,0. 882,0. 34,P > 0. 05),noise( t/z = 0. 699,-1. 309,-1. 834,-0. 702,P = 0. 487,0. 190,0. 067,0. 486,P > 0. 05) and SNR( t/z =-1. 131,-0. 858,1. 941,0. 098,P = 0. 258,0. 391,0. 057,0. 328,P > 0. 05) of RCA,LMA,LAD and LCX artery between TRO group and CTA group( P > 0. 05). CNR( t = 0. 531,P = 0. 598) of RCA revealed no difference between the two groups( PP > 0. 05).There was no significant difference in subjective scores of the coronary segments between TRO and the CTA group( P >0. 05). Conclusion TRO examination using 256-row Revolution CT can achieve comparable image quality and arterial enhancement of pulmonary trunk and ascending thoracic aorta against dedicated coronary CTA when using same amount of contrast medium.
