摘要
目的验证锥形束CT(CBCT)测量牙齿全长的准确率,探究全景片和CBCT测量前牙冠根比的差异性。方法将5颗解剖湿头颅标本拍摄CBCT后完整拔除上下前牙,使用电子游标卡尺测量解剖全长,在CBCT图像上测量每颗牙齿全长,对比两者验证CBCT测量牙齿全长准确率。选取30例同时拍摄了全景片和CBCT的患者,使用改良Lind法,分别计算前牙的全景片冠根比和CBCT冠状截面冠根比,通过SPSS 25.0进行配对样本t检验,探究CBCT和全景片测量冠根比的差异性。结果离体前牙解剖测量值和CBCT测量值差异不显著;全景片和CBCT测得的冠根比差异不显著,全长、根长差异均具有显著性,全景片测量冠根比的灵敏度为91.92%,特异度为92.07%,正确诊断指数即Youden指数为0.84。结论CBCT可以准确测量前牙长度;全景片测量前牙冠根比同CBCT测量结果无统计学差别,且具有较好的诊断价值。
Objective To verify the accuracy of measuring root length by CBCT,and explore the differences between the crown and root ratio(CRR) measured by pantomography and CBCT. Methods Anterior teeth were integrally extracted from 5 anatomical wet skull specimens after taking CBCT examination. The full length of the teeth was measured respectively by electronic vernier caliper and CBCT. Then 30 patients with both pantomography and CBCT images were selected,and improved Lind method was used to calculate the crown and root ratio from pantomography and coronal slice of CBCT respectively. At last,paired sample t test was conducted with SPSS25.0 to explore the difference between CBCT and pantomography in measuring CRR. Results The actual full length of anterior teeth was not significantly different from the length measured by CBCT. And there was no significant difference of CRR measured by pantomography and CBCT,and the differences in both the full length and root length were significant. The sensitivity and specificity of the CRR measured by pantomography were 91.92% and 92.07%,and the correct diagnosis index,namely Youden index,was 0.84. Conclusion CBCT can accurately measure the length of anterior teeth. There was no significant difference between the results of pantomography and CBCT in the measurement of anterior CRR,showing good diagnostic value.
