妊娠期高血压围生期行子宫动脉彩色多普勒血流监测及舒张早期切迹对妊娠结局的影响 被引量:44

Effect of ultrasound doppler flow monitoring of uterine artery and early diastolic incision on pregnancy outcome in perinatal hypertension

摘要 目的探讨妊娠期高血压疾病(HDCP)围生期行子宫动脉彩色多普勒血流监测及舒张早期切迹对妊娠结局的影响。方法选取120例HDCP患者为病例组,90例正常妊娠孕妇为对照组,比较两组子宫动脉血流动力学参数、舒张早期切迹发生率及妊娠结局的差异。根据病例组是否出现舒张早期切迹,分为无切迹组与切迹组,并比较两组妊娠结局的差异。结果病例组搏动指数(PI)、阻力指数(RI)和收缩-舒张流速比值(S/D)较对照组显著升高(P<0.01),舒张早期切迹发生率较对照组显著升高(P<0.01)。病例组孕妇身体质量指数(BMI)较对照组显著升高,孕周和新生儿出生体重明显低于对照组,胎儿窘迫、剖宫产和早产发生率较对照组显著升高(P<0.05)。相比无切迹组,切迹组孕周明显减小,新生儿出生体重显著降低,胎儿窘迫、羊水异常、早产和新生儿窒息发生率较对照组显著升高(P<0.05)。结论HDCP患者围生期行子宫动脉超声多普勒血流监测可有效预测并发症的发生,且子宫动脉舒张早期切迹在HDCP中具有良好的应用价值,发生舒张早期切迹的HDCP患者妊娠结局可能差于无切迹者。 Objective To analyze the effect of perinatal ultrasound doppler flow monitoring of uterine artery and early diastolic incision on pregnancy outcome in patients with hypertensive disorder complicating pregnancy(HDCP).Methods 120 patients with HDCP in our hospital from January 2017 to January 2019 were selected as the case group,and 90 normal pregnant women as the control group.The differences in uterine arterial blood flow parameters,incidence of early diastolic incisions and pregnancy outcomes between the two groups were compared.According to the presence or absence of early diastolic incisions in the case group,the patients were divided into the unincised group and the incised group,and the pregnancy outcomes of the two groups were compared.Results The ratio of pulse index(PI),resistance index(RI)and peak-systolic/diastolic-ratio(S/D)in the case group was significantly higher than that in the control group(P<0.01),and the incidences of early diastolic incisions were significantly higher than that in the control group(P<0.01).Body mass index(BMI)of pregnant women in the case group was significantly higher than that in the control group,gestational weeks and neonatal birth weight were significantly lower than that in the control group,and the incidence of fetal distress,cesarean section and preterm birth were significantly higher than that in the control group(P<0.05).Compared with the control group,the incidences of fetal distress,amniotic fluid abnormality,premature delivery and neonatal asphyxia were significantly increased in the incised group(P<0.05).Conclusion Perinatal ultrasound doppler flow monitoring of uterine artery in HDCP patients can effectively predict the occurrence of complications,and early diastolic indentation of uterine artery has good application value in HDCP.Pregnancy outcomes of HDCP patients with early diastolic indentation may be worse than those without indentation.
