摘要
目的探讨三维适形放疗和腔内放疗治疗宫颈癌的临床效果及对患者卵巢功能及生活质量的影响。方法选取2014年1月至2019年1月间西安医学院附属宝鸡医院收治的100例宫颈癌患者,按照治疗方式不同分为三维适形放疗组(48例)和常规治疗组(52例)。三维适形放疗组患者采用三维适形放疗和腔内放疗,常规治疗组患者采用传统放疗和后续治疗,比较两组患者的治疗有效率和生活质量,检测治疗前后雌二醇(E2)、促卵泡激素(FSH)和黄体生成素(LH)水平,检测卵巢阻力指数(RI)和搏动指数(PI)。结果三维适形放疗组患者总有效率为72.9%,高于常规治疗组的38.5%,差异有统计学意义(P<0.05)。治疗后,三维适形放疗组患者E2水平高于常规治疗组患者,LH和FSH水平低于常规治疗组患者,差异均有统计学意义(均P<0.05)。治疗后,三维适形放疗组患者RI和PI水平均高于常规治疗组,差异均有统计学意义(均P<0.05)。放疗后,三维适形放疗组患者卵巢功能减退率为22.9%,低于常规治疗组的53.8%,生活质量症状评分低于常规治疗组,差异均有统计学意义(均P<0.05)。结论三维适形放疗和腔内放疗治疗宫颈癌疗效更佳且对卵巢不良影响较小,患者生活质量更高。
Objective To study the efficacy of three-dimensional conformal radiotherapy plus intracavitary radiotherapy for cervical cancer and its effect on ovarian function and quality of life.Methods From January 2014 to January 2019,100 patients with cervical cancer who were treated at Baoji Hospital Affiliated to Xi’an Medical University were selected.According to the different treatment methods,the patients were divided into the three-dimensional conformal radiotherapy group(48 patients)and the conventional treatment group(52 patients).Thethree-dimensional conformal radiotherapy group underwent threedimensional conformal radiotherapy plus intracavitary radiotherapy and the conventional treatment group underwent conventional treatment and subsequent treatment.The treatment efficacy and quality of life were compared between the two groups.Levels of estradiol(E2),follicular-stimulating hormone(FSH),luteinizing hormone(LH),resistance index(RI)and pulse index(PI)were measured before and after the treatment.Results The overall efficacy was 72.9%in the three-dimensional conformal radiotherapy group,which was significantly higher than 38.5%of the conventional treatment group(P<0.05).The levels of E2 increased and levels of RI and PI decreased in the three-dimensional conformal radiotherapy group after treatment(all P<0.05).Levels of RI and PI were significantly higher in the three-dimensional conformal radiotherapy group than in the conventional treatment group after the treatment(all P<0.05).The rate for ovarian dysfunction was 22.9%in the three-dimensional conformal radiotherapy group which was significantly lower than 53.8%of the conventional treatment group(P<0.05).The score of symptoms were lower in the three-dimensional conformal radiotherapy group than in the conventional treatment group(all P<0.05).Conclusion Three-dimensional conformal radiotherapy plus intracavitary radiotherapy for cervical cancer is more effective than conventional chemotherapy and has less adverse effects on ovarian function,which can effectively improve the quality of life in patients with cervical cancer.
