心肌梗死溶栓危险指数与AMI患者PCI术后Gensini积分及在院转归的相关性分析 被引量:4

摘要 【目的】分析心肌梗死溶栓危险指数(TRI)评估急性心肌梗死(AMI)患者病情及在院转归的价值。【方法】前瞻性选择2016年3月至2019年1月本院收治的125例行急诊经皮冠脉介入术(PCI)治疗AMI患者,其中男66例,女59例,年龄38~83(57.38±10.61)岁,根据患者在院期间预后情况将其分为预后良好组(n=27)及预后不良组(n=98)。比较两组患者临床资料差异性,探讨TRI与AMI患者Gensini评分及在院转归的关系。【结果】预后不良组患者年龄、cTnI、CK-MB、TRI及Gensini评分显著高于预后良好组,LVEF低于预后良好组,差异有统计学意义(P<0.05)。Pearson相关性分析显示,TRI与cTnI(r=0.401,P<0.05)、CK-MB(r=0.529,P<0.05)及Gensini评分(r=0.547,P<0.01)呈正相关性,而与LVEF为负相关关系(r=-0.736,P<0.01),但与年龄(r=0.168,P>0.05)无明显相关性。ROC曲线分析显示,TRI预测AMI患者PCI术后在院期间转归不良的价值尚可,其曲线下面积(AUC)(95%CI)为0.81(0.73~0.89),TRI最佳预测界值(约登指数最大)为21.46分,该最佳诊断界值下的预测敏感性(95%CI)和特异性(95%CI)分别为0.83(0.75~0.92)、0.74(0.68~0.81)。【结论】TRI是潜在的评估AMI病情严重程度及在院预后不佳的指标,值得临床借鉴。
