摘要
目的探讨不同术式治疗早期子宫内膜癌患者的临床效果。方法采用随机数字表法将80例早期子宫内膜癌患者分为观察组(行腹腔镜手术治疗)和对照组(行开腹手术治疗),每组40例。比较两组患者的术中指标,手术前后的性功能、视觉模拟疼痛(VAS)评分,以及治疗期间并发症的发生情况。比较两组患者的远处转移、局部复发和阴道残端复发情况。结果观察组患者的手术时间、住院时间均明显短于对照组,术中出血量明显少于对照组,淋巴结切除个数明显多于对照组(P﹤0.01)。术后,两组患者的性功能评分均高于本组术前,VAS评分均低于本组术前,且观察组患者的性功能评分高于对照组,疼痛程度评分低于对照组(P﹤0.05)。观察组患者的并发症总发生率低于对照组(P﹤0.05)。观察组患者的远处转移发生率、局部复发率、阴道残端复发率均低于对照组(P﹤0.05)。结论腹腔镜手术治疗早期子宫内膜癌可减少患者的术中出血量,缩短手术时间和住院时间,有效缓解患者的性功能障碍和疼痛程度,提高术后生活质量,安全性较高。
Objective To investigate the clinical effects of two different surgical approaches for early endometrial cancer.Method A total of 80 patients with early endometrial cancer were randomly divided into study group treated with laparoscopic surgery(n=40)and control group treated with open surgery(n=40).The intraoperative measures,sexual function and visual analogue score(VAS)before and after surgery,as well as the incidence of complications during treatment were compared between the two groups.The subsequent distant metastasis,local recurrence and vaginal stump recurrence were compared between the two groups.Result The operative time and length of hospitalization of study group were shorter than those of the control group,the volume of intraoperative blood loss was less,and the number of resected lymph nodes was more in study group than in control group(P<0.01).After surgery,the scores of sexual function increased,and VAS scores decreased in both groups,with significantly higher score of sexual function and lower degree of pain noted in study group than in control group(P<0.05).Study group patients had lower overall incidence of complications versus those in control group(P<0.05).The distant metastasis rate,local recurrence rate and vaginal stump recurrence rate in study group were lower than those in control group(P<0.05).Conclusion Laparoscopic surgery for early endometrial cancer can reduce the volume of intraoperative blood loss,operative time and hospital stay,thus may effectively alleviate patients’sexual dysfunction and pain,improving their quality of life,demonstrating high safety.
